The ideal candidate will possess a strong background in nutrition science and dietetics, with experience in creating personalized nutrition plans and counseling patients on health maintenance and disease prevention. As a Nutrition and Health Guidance NP, you will play a pivotal role in empowering our patients to make informed decisions about their health through diet and lifestyle changes.

Key Responsibilities:
- Conduct comprehensive assessments of patients’ nutritional status, health habits, and underlying health conditions.
- Develop and implement individualized nutrition care plans to address specific health issues, such as diabetes, obesity, hypertension, and heart disease.
- Provide dietary counseling and education to patients and their families in both individual and group settings.
- Collaborate with physicians, dietitians, and other healthcare professionals to coordinate care and manage patient treatment plans.
- Monitor and evaluate patients’ progress and make adjustments to nutrition plans as necessary.
- Stay current with the latest research in nutrition and preventive health to provide evidence-based guidance.
- Participate in community outreach programs and wellness workshops focused on nutrition and healthy living.
- Document care in patient records and maintain confidentiality according to HIPAA regulations.

Qualifications:
- Current licensure as a Nurse Practitioner in the state of Georgia.
- Master’s degree in Nursing or a related field from an accredited program.
- Certification in Nutrition or a related specialty is highly preferred.
- Minimum of 2 years of clinical experience with a focus on nutrition counseling and health promotion.
- Excellent communication and interpersonal skills, with the ability to motivate and educate patients.
- Proficiency with electronic health records (EHR) and telehealth platforms.
