HEALTH COACH:
Join the Pacific Private Practice Network (P3N) team as a Health Coach. We are looking for a Medical Assistant, Nursing Assistant, Pharmacy Technician, Vocational Nurse, or other qualified healthcare provider. We are a private company with the primary focus to promote lifestyle change and collaborative care between patients and their physicians.
We are looking for a skilled and organized medical assistant or equally qualified individual, as mentioned above, to join our team as a Health Coach. You will perform a combination of various administrative and clinical tasks.
The Health Coach performs monthly care management for qualifying patients enrolled in one or more of our Health Coaching programs which include: Chronic Care Management, Remote Physiologic Monitoring, and Pharmacy Collaborative Care You can visit gotomyp3n.comfor more program details.
The Health Coach will be providing continuous care and education to the patient and/or family/caregiver(s) in adherence with a pre-established care plan. Care is provided under the supervision of the primary care team (i.e. Physician, Nurse Practitioner, and Registered Nurse).
This position will require the following:
Responsibilities:
1.Provide 20 minutes of enrolled patient care coordination once a month via phone. Further time may be required based on patient needs. There is an expectation to complete 2 or more encounters per hour worked.
2.Remain compliant with care plan and documentation requirements in the EHR.
3.Monitor adherence to care plans as well as assess the patient’s health status, goals, and progress.
4.Educate the patient and family/caregiver(s) about relevant community resources.
5.Assist with the identification of “complex” patients and refer to appropriate specialty educators or behavior health specialist based on medical condition.
6Establish continuity of patient care with the patient’s primary care provider, consulting specialty providers, facilities, and other external resources.
7.Attend weekly care team meetings.
Qualifications:
1.1 years' medical experience (preferred)
2.Patient/Customer Service Experience
3.Highschool Diploma
4.Proficient use of EHR/EMR… training will be provided on our EHR
5.CMA, CNA, pharmacy technician or applicable Certification
Candidate must live in Southern California and have reliable transportation.
If desired, after two-week training and probational period (60 days), employee may work from home.
No benefits included for part-time position. Benefits can be provided for full-time employee following probational period, if requested.
Competitive pay based on experience.
