Health coaching can potentially address several of the barriers to evidence based care including: limited access to care, poor coordination of care, distrust of and lack of engagement with the healthcare system, missed appointments, inadequate adherence to treatment, and poor patient-provider communication.
The iHealth Certified Health Coach will work closely with a Dietitian and Clinical Assistant in a fast-paced, technology-driven environment to provide patients with high-quality care regarding Hypertension, Hypotension, Diabetes, Pre-Diabetes, and Obesity. This position will be part of a wider-scope endeavor to test the feasibility and potential impacts of a health coach-facilitated and coordinated care plan.
The individual will receive extensive training at the beginning of employment to gain the knowledge and skills needed to work with patients in an individualized manner. The Health Coach will review patient records, gather relevant patient data, meet with referred and enrolled patients, and provide basic health education regarding diet and vitals to support the implementation of the recommendations accepted by the primary care clinician. The Health coach will consult with the Dietitian, Clinical Assistant, and contracted Primary Care Provider regularly, and act as a representative of the care team when following up with patients.
Review patient records and gather pertinent health metrics, including blood pressure, glucose levels, weight trends, and food diary insights, while facilitating and reviewing routine screening questionnaires with patients
Offer virtual coaching sessions to assist patients in setting, adjusting, and achieving their medical and lifestyle goals with foundational health education on Hypertension (HTN), Diabetes Mellitus (DM), Pre-Diabetes (pre-DM), and Obesity
Ensure guidance aligns with iHealth’s Care Standards, in conjunction with AHA and ADA recommendations
Follow up with patients to ensure the implementation of specialist recommendations endorsed by the primary care clinician
Assist the Clinical Assistant in patient onboarding, support, and engagement to enhance monitoring frequency and overall team interaction
Escalate cases requiring individualized Medical Nutrition Interventions to Registered Dietitians when clinical input is desired
Communicate on behalf of the team to engage professionally with clinicians, staff, and patients to establish positive relationships
Conduct patient outreach calls following provided scripts to schedule appointments, provide follow-up care information, and promote health education
Aid in administrative tasks such as data entry and scheduling when necessary
Bachelor’s degree in Health Sciences, Kinesiology, or a related discipline OR an AA with two (2) years of health and wellness-related experience and/or training is required
Certified Health COACH (NBHWC) Certification is required
Bilingual proficiency in Vietnamese is required
Experience in direct patient care procedures, including checking in patients, obtaining patient histories, taking vital signs, preparing charts, and assisting with medical examinations
Organized, tech-savvy, and highly driven with the knowledge, skills, and abilities to guide individuals in improving their diet and lifestyle
Independent and self-motivated with the experience of working with and appreciating a culturally diverse patient and staff population
