The Hospice Nurse Practitioner provides clinical practice oversight and management in accordance with state specific nurse practice law for patients in the hospice care setting.
Working under a collaborative practice agreement with the hospice Medical Director and team physicians the Nurse Practitioner collaborates with the attending physician, specialists and non-physician providers as applicable for the purpose of coordination of care to effectively meet the medical, psychological and spiritual needs of the patient.
This enables patients to live life as fully and comfortably as possible in an atmosphere where patients and families are able to share their thoughts, hopes and concerns.
Travel throughout your work day to multiple locations which may include private residences, assisted living and skilled nursing facilities
Provides effective communication to patient/family, team members, physicians, and other health care professionals
Assess, plan, implement and evaluate nursing care needs for the patient and their family utilizing the resources of the organization and community
Collaborate with an interdisciplinary team to promote patient and family choice, assure continuity of care, and enhance the quality of life of dying patients
Demonstrate critical thinking skills, decisive judgment, and the ability to work under minimal supervision
Work under physician direction to complete medical requirements while coordinating a whole person plan of care
Provide care according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act
Must be available to cover On-Call
