Ozarks Community Hospital in Rogersville, MO has an immediate opening for a full-time Community Health Worker (CHW) within our Medical Home department.

Our facilities include a hospital located in Gravette, Arkansas and a number of satellite clinics located in Southwest Missouri and Northwest Arkansas.

Ozarks Community Hospital takes great pride in providing patients accredited Patient-Centered Medical Homes through a dozen rural health clinics and as part of a broader range of specialty services located in its two provider-based clinics in Gravette, Arkansas and Springfield, Missouri. Specialty clinic services include Pediatrics, Women’s Health, Diabetes Care, Hepatitis C, Urology, Geriatrics, Pain Management, and the FLAG (Fatty Liver and Goals) Clinic.

Job Summary: Under the Manager of Medical Home, CHWs will work with the high-utilizer (frequent ER visits and/or multiple hospitalizations) subset of Primary Care Health Home (PCHH) participants, Chronic Care Management (CCM) or any at-risk OCH patient with a goal of stabilizing outpatient status and management and reducing avoidable hospital and emergency department use. CHW will help to bridge patients to resources to reduce social determinants of health that impact patients’ health and well-being. Provides informal counseling and social support, culturally appropriate health education, and basic screening services. Ability to perform duties consistent with Standards of Practice, goals and philosophy of Ozarks Community Hospital and the Employee Code of Conduct.The most significant duties have been included in this description. This does not preclude the assignment of occasional additional or developmental duties. The department reserves the right to modify this job description as needed to accurately reflect duties assigned.

Requirements:

  • High school diploma or equivalent.
  • Minimum of one (1) year’s medical office experience preferred.
  • Previous experience with Medicaid coverage preferred.
  • Previous experience conducting interviews preferred.
  • Must obtain CHW certification through an approved curriculum within 12 months of hire date.
  • Valid Driver’s License required. This position may require a Motor Vehicle Driving Record Background Check to be completed during the onboarding new hire process and throughout employment.
  • Must have a valid Class E license upon hire or Class E license must be obtained during orientation period for employees working in Missouri.
  • Current BLS certification upon hire or BLS certification must be obtained during orientation period or at the next available scheduled class. Throughout employment, current BLS certification must be maintained by renewal during renewal period or at next available scheduled class.
  • Current CARE certification upon hire or CARE certification must be obtained during orientation period or at the next available scheduled class. Throughout employment, current CARE certification must be maintained by renewal during renewal period or at next available scheduled class
