OVERTURE is Hiring a Health Services Coordinator Near Windsor, CO

Job Type: Full-time

Pay: $42,000 - $47,000

Minimum Requirements:

  • RN or LPN certification/licensure
  • Must have solid computer and organizational skills with the ability to manage multiple projects at one
    time.
  • Must be able and willing to work some hours outside of normal working hours to respond to emergencies
    or to otherwise meet the needs of the people we serve and their families.
  • Must have consistent access to reliable transportation that will allow for quick response in an emergency.
  • Must have compatible smart phone to use RingCentral mobile app on.
  • Must possess a valid Colorado Driver’s License and proof of insurance per Overture insurance guidelines.
  • Must pass background check and other screening per Overture requirements

Preferred Job Qualifications and Competencies:

  • Problem solving skills with ability to respond to problems calmly and independently.
  • Strong organizational skills with attention to detail and strong follow-through on assigned tasks.
  • Excellent communication skills, both spoken and written.
  • Solid computer skills with ability to write/format letters, memos, develop appropriate health related forms and perform medication research on the internet.
  • Organized and be able to work irregular hours as need by caseload.
  • Experience working in the field of mental health or with people with Intellectual/Developmental
    Disabilities preferred.
  • Solid nursing case management skills.
  • Maintain professional appearance and a positive attitude whenever representing Overture.
  • Exemplify Overture’s vision, mission, and values and maintain good attendance as outlined in the
    attendance policy.
  • Maintain ongoing compliance with all required HR documentation and online/instructor-led trainings.
    Completes all trainings and documentation no later than the due dates given.
  • Passion and commitment to serve persons with intellectual and developmental disabilities with dignity
    and acceptance of Overture’s Person-Centered philosophy.

Essential Functions and Responsibilities:

  • Effectively coordinate necessary medical care, required appointments, and maintain documentation of
    health services according to state regulations for the individuals we serve.
  • Provide consultation to the individuals we serve, host home providers, and Overture program coordinators regarding health care issues related to each individual in services.
  • Create protocols and provide training and supervision in the implementation of these protocols.
  • Provide support and training for managing medication administration and monitoring for side effects.
  • Monitor MARS monthly for proper documentation.
  • Perform self-medication assessments with continued supervision and monitoring as needed and at
    minimum, quarterly.
  • Ensure psychiatric appointments for individuals on psychotropic medications are completed at least
    annually. Train on identifying side effects and needed lab work for monitoring as appropriate.
  • Coordinate physical, dental, visual, neurological, audiology and other medical appointments for
    individuals we serve according to state regulations.
  • Attend medical, psychiatric appointments, and individual meetings as needed when nursing support is
    needed in an emergency.
  • Ensures needed therapies are completed as ordered and that all DME is properly maintained per state
    regulations.
  • Conduct home visits as assigned.
  • Conduct QMAP classes, G-tube classes and Nursing Orientation classes as directed.
  • Effectively interface with a variety of individuals including health care professionals, families, individuals we serve, and staff members from various service agencies, host home providers, and Overture staff.
  • Develop crisis prevention strategies and strategies for the management of medical crises as they occur.
  • Work with the Residential and Day Services team to problem-solve and carry out health support actions.
  • Review incident reports for follow-up including providing appropriate recommendations and training.
  • Administer training and education for new and current staff members and HHP’s as outlined in the
    Overture guidelines. Provide additional training and document when issues or problems arise that can be
    resolved with further training.
  • Participate in Health Services meetings and agency staff meetings as assigned.
    Interface with Medicaid or Medicare programs as needed.
  • Performs other such duties as assigned within the scope of the position, as well as those reflective of
    professional experience, education, and ability.
