Clinical Site Coordinator is a Licensed Behavioral Health Clinician that facilitates collaboration between OMHC therapists SUD Facility counselors and therapists, school-based therapists, PRP providers, psychiatric staff, peer support staff, psychology department, Tele-medicine staff, Tele-counseling staff and administrative support staff. Clinical Site Coordinator is a member of the Clinical Leadership Team: Attends weekly meetings, offers creative solutions to policy and procedures that adds to the quality and quantity of service provision as developed and implemented by Program Director.
License required LGPC, LCPC, LGADC, LCPC, LMSW, LCPC. LCSWC
Overall Duties & Responsibilities:
1. Demonstrates positive professional regard for all patients and utilizes supervisory resources
when necessary. Having an empathetic and supportive attitude towards all consumers and families
we serve is a must.
2. Engages clients in the therapeutic services screening, assessment, individual counseling,
group therapy, family therapy. Therapeutic process as evidenced by patients’ satisfaction
with treatment designs and implements clinically appropriate treatment plans utilizing
evidence based practice techniques to create goals that are individualized, observable, and
measurable.
3. Communicates effectively both verbally and in writing.
4. Maintains accurate documentation in client records that is in compliance with all applicable
state and federal regulations and agency clinical standards.
5. Completes documentation required for billing including Pre-Authorization Requests within
required time frames monitoring clinical staff with chart audits.
6. Initiates and encourages Clinical teams Active participation in clinical team meetings and
other appropriate meetings as scheduled. Refers and coordinates consumer care with
multidisciplinary clinical treatment team, PRP, Psychiatric and Testing services.
7. Demonstrates knowledge of cultural norms and self-awareness of values, biases and
Stereotypes. Provides training to Clinical direct care
8. Maintains knowledge and awareness of available community resources. Makes appropriate
referrals for needed services and assists consumers in accessing appropriate resources.
Willingness to attend trainings and workshops and to incorporate new skills into
professional action.
9. Monitors the clinical staff’s Initial and follow up contact with consumers and families
requesting information, services and community referral sources, scheduling clients with the
appropriate service and provider.
10. Monitors and schedules new consumer intake assessment appointments and explaining
new consumer policies, procedures, clinic paperwork, and human rights in a clear simple
and professional manner. Provides Annual training for clinical staff .
11. Monitors the scheduling Internal/External Referrals, Monitors Follow up with EHR
appointment with requests and clinical appointment management as well as
communication with the referring sources. Monitors EHR communication utilizing reports in the EHR to ensure communication in services with signed release from consumer is
documented in a timely manner.
12. Monitors the open communication with the Wellness Center Receptionist Staff,
Counselors, Operations Manager, Executive Staff and Clinical Director related to
consumer direct care services and clinical operations in weekly staffing's.
13. Monitors the maintenance of the s pre-registration/Screening forms and ensure documents
have been scanned to the EHR for tracking and updating Intake documents for scheduling
needs.
14. Monitors and assists consumer in accessing EHR patient portal, forms as well as tracking
intake forms for prospective consumers, coordinate and record intake email for
correspondence from providers/consumers, referral sources
15. Monitors new consumer registration input and new consumer reported information related
to insurance information, Next of Kin, gender Identity, sexual orientation, demographics,
status, medical history, current medications goals, and advance directives in EHR
16. Monitors the process all incoming intakes – gathering additional information as needed
documenting information in the EHR send notices to the Clinical Director for Each New
Consumer provided an Assessment appointment
17. Monitors, Completes and receives clinical staff walk-in intakes, assessments and referrals to inpatient treatment as needed- Schedule screening and assessments. Monitor all Same Day Scheduled Urgent Screenings and Assessments to inform intake when walks ins occur live in EHR and by phone for supervision check with Wellness Center Receptionist (front
desk) on room availability.
11.Monitors Clinical staff responsible for maintaining new intake provider grid for inpatient
referrals are up to date; have working knowledge of all services, providers and staff to
coordinate warm handoff to assigned counselor
12. Monitors Orientation of new consumers and families services available, assist
consumers and families and clinical staff with transferring between providers as needed
when transitioning to different levels of care
13. Monitors clinical staff, direct care staff responsible for keeping records of internal referrals
and transfers document in EHR
14. Monitors Verification of insurance for consumers and families obtaining insurance
authorizations and contract agreements
15. Performs and monitors clerical duties as needed and assigned related to clinic needs
Documentation, Clinical Services, Community Outreach needs assisting the Clinical
Director
16 Performs and monitors clinical staff on Educating consumers, families, community referral
sources, stakeholders on EHR set-up and completion of forms and HIPPA compliant
communication in accessing communication agency EHR.
17. Provides and monitors Staff community presentations and support as
community liaison promoting services provided by agency under the direction of Clinical
Director
18. Other Duties may be assigned as needed based on agency need.
POSITION REQUIREMENT:
Master's Degree in a behavioral health /human services field required. Clinical Supervision experience preferred and/or 5 years or more of experience working with consumers diagnosed with behavioral health diagnosis, Substance Use Disorders and Addictions. 3 years or more experience in administration required. Maintains requirements of appropriate state licensure boards. Licensed or certified as a behavioral health or counselor preferred.
