Ounce of Care is Hiring a Community Health Care Coordinator Near Washington, DC

Job Description:

We need help on a part-time, flexible basis to help with resident care coordination. In this remote role, you will support our team of on-site Community Health Leads (CHLs) by assisting residents over the phone. This can include conducting a resident intake & needs assessment, getting residents enrolled with Ounce, connecting them with healthcare services & scheduling appointments, and completing benefits applications.

You will work closely with our Community Health Leads, and represent Ounce within the community. You’ll help to build trust and respect with residents, identify their needs, and work with our team to make sure they get access to high-quality services that promote health & wellness.

Requirements:

  • Flexibility to work variable hours between 8 am - 5 pm ET, M-F (15 - 25 hours / week in total).
  • Experience with DC healthcare and benefits systems, including familiarity with applications and processes such as Medicaid recertification, SNAP, LIHEAP, SSI, and other benefits.
  • Experience with outreach to healthcare providers and/or patients.
  • Comfortable with data entry to record work being done for residents, including referrals, in our technology platform.
  • Candidates who are bilingual and fluent in Spanish are also strongly encouraged to apply (though we are also looking for English-speaking applicants).

Compensation:

  • $28 per hour
