General Statement of Duties:

The HIM Specialist II is responsible for ensuring all medical documentation is scanned or uploaded into the EMR The Specialist II is responsible for direct patient communication including answering and directing patient questions. The Specialist II manages retrieval of information to be placed in the patient’s chart for appointments that are scheduled in advance according to the physician guidelines.

Essential Duties:

  1. Processes documents from HIM fax queue to index into Allscripts as well as PT documentation in Docuware for those participating providers, faxing PT notes to appropriate providers
  2. Reviews Upcoming appointment schedule and/or surgery schedule in the portal for providers to ensure proper documentation is in the chart, notifies patient service teams and/or PASS/PAT team of any issues with chart prep prior to appointment, and retrieves records from other facilities that cannot be found in current documentation systems.
  3. Gather/Print Quarterly QA records
  4. Assist the patient in the completion of the required release of information authorization form in accordance in HIPAA guidelines. Determine the necessary escalation of patient requests to the disability team and/or copy service. Responding to walk in patients requesting release of information. Logs information provided by patient into ROI tracking system.
  5. Responsible for all copying of patient image reports. Collaborates with copy service to fulfill requests.
  6. Responds and processes requests from physician offices and Stat or continuing care patient and provider on a priority basis and faxes information to the appropriate location.
  7. Responds to all requests.
  8. Responsible for the uploading of CD’s into the EMR that have been brought into the facility by patients and/or providers.
  9. Proficient in HIM Specialist I duties
  10. Assists with Mailroom responsibilities, including making all rounds to collect medical documentation from all areas of the hospital and clinic.

Requirements:

  • High school diploma or GED required; specialty training beyond high school preferred
  • 2-4 years of related experience required; 4-6 years of experience preferred
