ORTHOPAEDIC ASSOCIATES OF ALLENTOWN is Hiring a Clinical Patient Care Navigator Near Allentown, PA

Summary: Under direction of the Director of Operations, the Patient Navigator functions on the multidisciplinary team as an advocate, interpreter, and educator for patients. The Patient Navigator coordinates patient care through the continuum in collaboration with the multidisciplinary team. Serves as the intermediary between the patient and care provider, responsible for patient education, scheduling care, post treatment follow-up, etc. Serves as a liaison throughout the practice regarding services provided for this unique patient population. Serves as a single point of contact for referring providers and patients. The role of the Patient Navigator is crucial to increasing patient and provider engagement and establishing a continuum of care including increasing internal practice utilization.

Essential Duties and Responsibilities

  • Provide clinical expertise with orthopaedic clinical knowledge.
  • Coordinate initial and ongoing care as the single point of contact/resource for patients, staff, and providers to resolution with improvement feedback to the company as appropriate.
  • Provide patient care navigation services throughout the continuum of care in all of OAA’s services and locations including immediate care, office-based, surgical care, ancillary services and any other services OAA provides or will provide.
  • Act as a patient advocate and educator for patients, visitors, and OAA staff at the direction of the Director of Operations.
  • Provide accurate, on-going documentation of navigation and care coordination activities on each patient within the EMR.
  • Facilitate communication between patient and healthcare provider(s)
  • Eliminate barriers for patients to optimize care and increase/expedite access in a measurable methodology.
  • Collect, track, trend, analyze and report data on the results of your improved outcomes.
  • Promote orthopaedic evidence-based clinical practice.
  • Patient and Visitor liaison for needs/problem solving.
  • Evaluates the care given and makes recommendations to develop action plans for improvements.
  • Serve as a member of the Safety and Quality Committee
  • Back-up support for clinical areas including medical assisting and DME
  • Other duties as assigned.

Qualifications:

  • Certified LPN, Medical Assistant, or Physical Therapy Assistant
  • 3 years clinical experience, Orthopedics and/or PT/OT preferred.
  • Proficient with medical terminology
  • Patient education experience
  • Proven working relationship with medical providers.
  • Excellent organizational skills, able to manage a full suite of tasks and manage across numerous priorities and to established deadlines.
  • Excellent communication and interpersonal skills.
  • Employs empathy, patience, understanding, courtesy and kindness to all patients, their families, physician and other practice staff.
  • Ability to effectively communicate with team members in a positive/constructive manner to evolve process and policy.
  • Able to focus on long-term goals while managing the day-to-day operations
  • Exercises initiative, creativity and courage to act in the best interest of the patient.
  • Confidence to interact with stakeholders at all levels, including patients/family, co-workers, providers and administration.
  • Ability to create and/or take initiative from concept to results, as well as to sustain the change.
  • Proficient in MS Word, Excel, and PowerPoint
  • Knowledge of EHR systems, Athena, Ambra, and WebPT.
