Mobile Medicine! Hiring Nurse Practitioner or physician assistant for Florence Oregon
Mobile Clinical Care, includes in home visits, visits in the mobile medical unit and telemedicine visits during travel to patient homes – 30 Hours per Week
Driver of Medical Van will be either a Community Health Worker or Expanded Practice Dental Hygienist for Mobile Medical Routes.

Oregon Integrated Health is an Integrated Primary Care Medical Home that is hiring for a Full Time Primary Care Provider for our mobile medicine unit to reach home bound patients. One day a week clinical care at our Florence office.
Our Integrated Medical Home in Florence currently has a team of Family Nurse Practitioner, Licensed Professional Counselor, Medical Assistants, a Community Health Worker, Front Desk and our Senior Director of Clinical Operations. Our patients have access to Quest Laboratories directly in our facility. We are also expanding our space to initiate the Vaccine for Children program by September 2023.
Reaching the Homebound Patient Florence Coastal Oregon, Lane County is a designated Health Professional Shortage Area (HPSA). This geographic region is populated with approximately 40% persons over the age of 65 and more than 17% of the residents living in poverty. From the US Census Bureau, the major population belongs to rural areas. Even with the few medical clinics and hospitals, the number of patients that are underserved overwhelm the current structure. A mobile medical van that incorporates telemedicine as well as medical transportation will play a vital role in connecting patients to clinicians at a lower cost and with a wider reach.
Primary care services would involve a clinical consultation with a patient using live interactive video, in home visits or visits in the mobile van – depending on the patient and the appointment needed. Mobile Medical Vans that provide home assessments, access to a community health workers and Telemedicine are particularly helpful in rural areas, where the shortage of healthcare providers has led to a lack of accessibility to basic healthcare. Oregon Integrated Health (OIH), established in 2011 in Portland and expanded in 2015 in Eugene with the Expansion of Primary Care Grant from Trillium and later in 2017 in Florence, 2021 in Salem. Our business has developed a foundation of workflows, procedures and policies to successfully meet the needs of Medicaid and Medicare members. Our intention and commitment is to grow in this rural community and expand the delivery of primary care through the use of mobile and telemedicine services.
