Transplant/Urology/Plastics (4A) is an acute care unit specializing in solid organ transplant (kidney, pancreas, and liver), complex plastic reconstruction surgeries, urology, and nephrology diagnoses and related surgeries. The patient population ranges from 18 years of age to the elderly.
The Certified Nursing Assistant is part of the multi-disciplinary team providing care to OHSU patients. The Certified Nursing Assistant provides direct or general nursing care to patients from admission through discharge under the direction of a licensed Registered Nurse and within the scope of practice defined by the Oregon State Board of Nursing. They assist the patients, patient’s families and friends, nursing staff, and other OHSU personnel to assure the delivery of comprehensive quality care in a safe, confidential and comfortable environment.
KEY RESPONSIBILITIES: Essential functions indicate those key responsibilities that meet one or more of the following descriptors: (1) the position exists to perform the function, (2) the number of employees available to perform the function is limited, (3) the function is so highly specialized that the person is hired for his/her expertise or ability to perform the function. The percentage of duties must equal 100%
1. Assist with the admitting and discharge
2. Obtain accurate vital signs and weight. Gather and secure patient valuables
3. Complete all unit specific CNA documentation in the patient’s medical record related to: admissions, daily care, vital signs, weight, intake and output, discharge, etc.
4. Ambulate patients, provide aides as directed for safety and notifies nurse of additional needs
5. Respond to call lights, assure that call light are within reach at all times. Seek assistance of the nurse or other health care personnel as needed
6. Assist with or lifts/transfers patients and perform other position requirements safely
7. Perform all duties necessary to assure delivery of prescribed diet and to meet all dietary needs
8. Empty commodes, bedpans, urinals, foley catheters, colostomies, urotomy, and other drainage receptacles/containers as directed. Collect and test specimens as directed (urine, stool, other)
9. Assist the patient with or perform basic hygiene
10. Assist with turning/positioning patients as indicated or directed. Assure good body alignment, comfort, range of motion (ROM), and support
11. Assume responsibility for professional/job related development, knowledge of advances in technology, use of new equipment and appropriate application of hospital systems. Disinfect or discard equipment/supplies appropriately
12. Provide and obtain shift reports with appropriate personnel when necessary or as directed
13. Incorporate quality improvement practices into performance
14. Incorporate the principles of OHSU’s Mission Statement into performance
15. Understand the teaching and research missions of OHSU Hospitals and Clinics
16. Support patient and family centered care across the continuum through service coordination
17. Perform staff member requirements as outlined in the Compliance Section of the Core Competencies
18. Adhere to the OHSU Code of Conduct
19. Perform requirements for Staff Members as outlined in Compliance Roles and Responsibilities in OHSU Hospitals and Clinics:Carry out job responsibilities in an ethical, effective and professional manner.Report potential compliance problems using the reporting processes as outlined in the Code of Conduct.Model ethical and appropriate behavior, professional standards, compliance, and personal integrity.Participate in training programs.
Understand and comply with the Code of Conduct, OHSU and departmental policies and procedures, applicable rules and regulations.
20. Screen for pain and comfort care needs and notify the appropriate health care team member, adhering to the OHSU policy for Pain Management

Basic math skills
Basic medical terminology skills
Registrations, Certifications and/or Licenses:
Oregon State CNA license or CNA 2 (Acute Care) certification
BLS certification issued by the American Heart Association (AHA)
