In this role, you’ll provide support to participants by providing case management and connecting them to housing relocation and stabilization services and resources, with the goal of obtaining and sustaining permanent housing. Schedule: Full-Time, Monday-Friday, 10am-6:30pm

Highlights of the position:

  • Works with people experiencing homelessness to evaluate housing barriers, needs, and goals. Evaluates participates through housing assessment and prioritization and refers to appropriate housing intervention.
  • Provides support to participants in their search for decent, affordable, and appropriate housing. This support includes but is not limited to:
    • Assists in completing applications, interpreting leases, understanding tenant rights and responsibilities, and more.
    • Assists in developing and adapting support and action plans.
    • Assists participants in building social skills that will enhance the acquisition and retention of safe, affordable housing.
  • Establishes and fosters relationships with housing programs, landlords, and property managers, and other shelter providers in the community to ensure adequate housing options for participants. Coordinates efforts to ensure participants receive available services and support needs tailored to fit their individual needs.
  • Works within teams to ensure Hope House program services are mission-focused. Continuously looks to seek improvement and implement best practices.

Education and Experience:

  • Experience in working with individuals experiencing homelessness required
  • Bachelors in Human Services or related field preferred
  • Case Management experience preferred
  • High School Diploma or equivalent required.
  • Having a valid driver’s license with an acceptable driving record and/or an appropriate length of driving experience for insurance purposes is required. Any concerns raised by the results of a driving record check will be subject to an individualized review to determine whether the infraction is relevant to the individual’s ability to perform the responsibilities of the position, in accordance with PCHC’s Automobile Safety and Background Check policy.
  • Will have up-to-date training and certification in Healthcare Provider Basic Life Support issued by American heart Association, American Red Cross, or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire.
