The Licensed Vocational Nurse (LVN) will operate as a Discharge Planning Coordinator. The LVN is responsible for the discharge of patients and following pre-established admission criteria in collabartion with RN Case Managers and other staff. You will work closely with the Social Worker and must be a naturally compassionate person.
Qualifications
- Must have an active license for the state of Texas in good standing as either an LVN or an RN
Responsibilities
- Ensure compliance with patient's plan of care
- Collaborate with multiple departments
- Coordinate and execute timely discharge plans for a designated caseload of patients.
- Ensure a safe transition.
- Work closely with the Social Worker
- Report any issues to management.
