Reports to: Director of Nursing, ADON and/or Charge Nurse

Supervises: None

Classification: Non-Exempt Hourly

Position Description

Responsible for delivering rehabilitative and therapeutic services ordered and supervised by licensed medical staff and in accordance with accepted standards of practice, state and federal regulations and licensing requirements. Assists residents to achieve or maintain their highest practicable level of ADL function through consistent, structured and controlled re-teaching and retraining techniques to promote increased participation in activities of daily living. Principal Responsibilities •
Provides restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc. within the scope of the training; Utilizes restorative techniques including verbal cueing, task sequencing, hand over hand, prompting, guarding, etc.; Promotes greatest degree of independence possible for residents.
• Assists residents with their physical needs including eating, bowel, and bladder training, and assistance following incontinence but encourages residents to perform these functions themselves to their capacity or ability; Participates actively in retraining programs for these functions.
• Observes and reports on resident’s conditions and documents observations as required. Contributes knowledge of the resident’s conditions/needs to resident care plans; Documents restorative exercises and weekly summaries as required.
• Assist in the care and maintenance of therapy area and equipment. Assures that equipment is clean and functional.
• Assists with lifting, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., in keeping with specific resident safety needs; Assists residents in preparation for and transporting to appointments, activities and social programs
• Participate in meal service including serving, clearing, and cleaning the dining room; Assures that residents are positioned appropriately for meals; Assures that appropriate utensils are within reach of residents; Serves food trays and assists with feeding as indicated; Delivers and documents all a.m., p.m., and bedtime snacks and supplements; Provides hydration to all residents according to center policy.
• Checks residents in section regularly; Answers call light promptly; Follows through with resident problems or needs by delivering care or reporting to supervisor depending upon level of ability and nature of resident need.
• Maintains resident stability by checking vital signs and weight; testing urine; recording intake and output information.
• Provides resident comfort by utilizing resources and materials; transporting residents; answering resident call lights and requests; reporting observations of the resident to nursing supervisor.
• Documents actions by completing forms, reports, logs, and records.
• Protects company’s integrity by keeping resident information confidential.
• Receives assignment from supervisor and performs duties in accordance with each resident’s detailed plan of care, providing care to residents who are presently receiving restorative ADL nursing interventions; Reviews care plans routinely to determine if changes in the residents’ care routines have been made.
• Participates in and receives nursing report upon reporting for duty and during shift change.
• Receives training from therapists and supervisors on the restorative techniques and performs techniques.
• Maintains knowledge of resident’s condition at times. Notes significant changes in the condition of residents and takes appropriate action; Identifies emergency situations and responds with appropriate measures.
• Takes and records temperature, pulse, respiration, intake and output; Reports resident incidents to immediate supervisor; Communicates requests and problems to appropriate personnel; Takes BPs upon request by Supervisor.
• Observes safety needs of residents including use of bedrails, rail pads, and other protective supports when ordered; Identifies safety and security hazards and reports to appropriate personnel.
• Assists resident with personal care, including nails, hair, oral hygiene, and dressing following Standard Precautions Encourages resident to do as much as he or she can do him or herself; Cares for resident’s prosthesis as required; Maintains resident privacy when providing care.
• Prepares residents for bed rest; Gives special care to residents in casts or traction such as monitoring comfort, skin color, and temperature. Moisturizes resident’s skin; Applies deodorant.
• Follows policies and procedures related to body mechanics, transfer, body positioning, and mobility as outlined in the nursing policies and procedures manual; Repositions residents as needed using supportive devices when necessary (handrails, pillows, etc.) and reports the same.
• Reports and documents self-care ability, assistance needed and; resident’s progress or lack of progress. Assists in resident’s transition from prescribed O.T., P.T., S.T., to restorative program and from restorative program to maintenance by CNA.
• Other duties, responsibilities and activities may change or assigned at any time with or without notice.

Direct Care Responsibilities

• Charts progress notes in an informative, factual manner that reflects the care administered as well as the resident’s response to care.
• Follows established procedure for charting and reporting all reports of incident/accidents (residents & employees).
• Identifies and reports changes in condition to charge nurse, supervisor, ADON and/or DON.
• Identifies and reports skin changes and follows the Company skin management protocols.
• Follows established procedures for restraint reduction.
• Assists with meal service; Identifies residents at risk for nutritional problems and implements preventative measures; Checks items and food that is brought in by family members to assure it complies with resident’s orders, dietary needs, and safety; Monitors residents with significant weight loss and gain and reports this to the physician and dietary.
• Completes documentation to reflect progress or lack of progress of residents participating in Quality of Life programs.
• Accurately completes and is familiar with the forms used throughout the resident’s chart.
• Participates (as appropriate) in the investigations related to any alleged abuse.
• Ensures that the progress notes are reflective of the care plan and that the approaches on the care plan are being followed.

Special Nursing Care Responsibilities

• Demonstrates knowledge and ability to implement: Positioning techniques; Transfer techniques; ROM exercises.
• Demonstrates knowledge and ability to implement: Emergency procedures.
• Demonstrates integrity and ability to delivery service standard for bedside manner.
• Inventories and stores residents’ personal possessions as instructed.
• Participates in care conference as indicated and follows individual resident care plans.
• Identifies and reports abnormal skin conditions. • Assists with application of adaptive devices.
• Demonstrates knowledge and ability to provide daily indwelling catheter care; Demonstrates knowledge and ability to provide perineal care. General Responsibilities • Attends and participates in scheduled training, educational classes, and meetings to maintain current certification as applicable; Attends and participates in in-service training as mandated by regulatory agencies and company policy.
• Adheres to work schedules in completing and performing assigned tasks; completes all assigned tasks in a timely manner as instructed by the direct supervisor.
• Meets acceptable center attendance requirements; notifies facility before late arrival to or absence from work according to established center policy.
• Cooperates with other facility personnel to ensure that services can be adequately maintained to meet the needs of residents; creates and maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.
• Adheres to established employee policies.
• Adheres to established dress code.

Customer Service Responsibilities

• Models customer service principals throughout the center and promotes appreciation of our customer’s needs.
• Treats residents, residents’ family members, visitors and fellow employees with courtesy, respect, and dignity. 
• Presents professional image through appearance, behavior and speech.
• Explains procedures to residents before administering services.

Residents Rights

• Understands, upholds and promotes the rights of the residents.
• Ensures residents can exercise rights without interference, coercion, discrimination, or reprisal from the facility.
• Ensures protected health information is kept confidential.
• Ensures resident concerns/complaints are responded to with tact and urgency.
• Reports allegations of resident abuse, neglect and/or misappropriation of resident property.
• Coordinates effective communication with Residents and Companions by assuring all arrangements for providing interpreters and/or other auxiliary aids and services needed by Residents and Responsible Party are made, including scheduling interpreters through outside agencies and scheduling staff interpreters, if any; and ensuring appropriate maintenance, repair, replacement and distribution of auxiliary aids.

Safety and Sanitation

• Reports observations concerning structural, equipment, and furniture defects and malfunctioning to appropriate personnel.
• Follows established safety policies and procedures.
• Identifies and corrects safety hazards; reports safety concerns as required.
• Observes safety needs of residents as indicated in care plan.
• Wears and/or uses safety equipment and supplies when indicated and properly trained to use.
• Demonstrates job-specific knowledge of fire and disaster preparedness during drills or actual situations.

Staff Development

• Attends and participates in scheduled in-service training, educational classes and meetings to maintain current certification as applicable and as managed by regulatory agencies and company policies.
• Ensures that small group meetings are held per company policy.
• Communicates new Policy & Procedures and regulations to staff to ensure compliance.

Qualifications

• Active CNA certification in the state of practice, required.
• Successful completion of state approved Restorative Nurse Assistant Training Program; Name must appear on State Registry as Nurse Aide in good standing.
• Prior experience as Restorative Nursing Assistant in a skilled nursing facility preferred.
• Current BLS/CPR certification (All SNFs; ALFs as required by state).

Travel

• Minimal. May escort residents to outside appointments.
