Three Rivers Hospice, Inc. is currently seeking a competent LPN to case manage hospice patients. LPNs work under the direction of the RN to assess, implement, and evaluate delivery of quality nursing care to our hospice patients. LPNs work with the Interdisciplinary team and attend IDG meetings every 14 days. Full benefits for full time employees including paid sick time and mileage reimbursement.
