Your Job

The Licensed Vocational Nurse (LVN) will operate as a Discharge Planning Coordinator. The LVN is responsible for the discharge of patients and following pre-established admission criteria in collabartion with RN Case Managers and other staff. You will work closely with the Social Worker and must be a naturally compassionate person.

Qualifications

  • Must have an active license for the state of Texas in good standing as either an LVN or an RN

Responsibilities

  • Ensure compliance with patient's plan of care
  • Collaborate with multiple departments
  • Coordinate and execute timely discharge plans for a designated caseload of patients.
  • Ensure a safe transition.
  • Work closely with the Social Worker
  • Report any issues to management.
