For more details check us out here http://www.ensignbenefits.com/ Benefits eligibility for some benefits dependent on full time employment status Duties: • The Case Manager facilitates patient care by assessing patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress • Responsible for clinical and financial assessments of patients prior to admission • Facilitates and actively participates in interdisciplinary planning of clinical care of patients, and assists with outcome report completion • Provides outcome data to payers and Physicians on as needed basis • Serves as care liaison between patient, physician, care team members, and discharge planner by coordinating, monitoring, and communicating patient’s progress • Assists with coordination to most appropriate care level • Initiation and implementation of plan modifications as necessary through monitoring and re-evaluation to accommodate changes in treatment or progress • Communicates discharge information to physician and payer with outcome data, as appropriate Qualifications: • RN or LVN License is required • Case Manager experience is required • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public • Must be knowledgeable of nursing and medical practices and procedures EOE/Minorities/Females/Veteran/Disability Back to Jobs We are a provider of skilled nursing and rehabilitative services and offer a work environment where our teams are empowered to achieve superior clinical outcomes and high resident/family satisfaction ratings.
