One Health is expanding our services in Sheridan and is looking for a Registered Nurse to join our team!

You: A Wyoming-licensed Registered Nurse (RN), solid in clinical competency, and committed to delivering and promoting physical and mental health care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. You enjoy interacting with patients and other health team members while maintaining high standards of professional nursing.

COMPENSATION: $30.40 - $32.38/hr, DOE

EMPLOYMENT TYPE:  Full-Time, 40 hours/week

AVAILABILITY: Mon-Fri 

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

  1. Triage patients, collecting vitals, review of systems, health history, and medication reconciliation, PHQ-2 and PHQ-9, GAD 7 information.
  2. Documents data into Electronic Health Record in accordance with established Patient Centered Medical Home (PCMH), Uniform Data System (UDS) and Meaningful use measures.
  3. Provides basic education information to patients and family, focusing on self-care appropriate to their age and identified learning considerations and needs.
  4. Administers medications per provider orders.
  5. Anticipates and prepares examination room with equipment and supplies required for the visit.
  6. Assists provider with examination and procedures per request.
  7. Performs lab testing including quality controls, maintains all laboratory/diagnostic test logs, and reports all critical lab values immediately.
  8. Administers vaccines to patients in accordance with current vaccine schedules.
  9. Participates in huddle with primary care team.
  10. Handles telephone encounters and medication refill requests for primary care team.
  11. Effectively uses the electronic medical record for charting patient nursing visits and telephone contacts.
  12. Keeps client medical records updated for reviews, audits, and contact purposes.
  13. Participates in quality assurance/quality improvement projects as needed or requested.
  14. Works as a team member with the primary care teams.
  15. Communicates clearly, concisely, and accurately, creating a working climate that provides growth and job satisfaction of personnel.
  16. Uses professional courtesy when communicating with patients, team members, other co-workers, and community members.
  17. Contacts patients regarding lab results.
  18. Identifies patients for specialized treatment services, and groups.
  19. Identifies health literacy needs for patients and provides proper health education materials.
  20. Upon request, provides brown bag medication review with patients.
  21. Develops and/or identifies appropriate patient education materials for use with patients and families.
  22. Assists with outreach activities as needed. This may include staffing health fairs, taking blood pressures, blood glucose testing.
  23. Works collaboratively with other health center staff in meeting accreditation and other regulatory and/or funding requirements.
  24. Adheres to all One Health Policies and Procedures.
  25. Participates in performance optimization and risk management activities as required.
  26. Cooperates and abides by all elements of One Health’s Compliance Program.
  27. Provides exceptional customer service and models the mission of One Health when interacting with internal and external stakeholders promoting a positive image for the health center.
  28. Performs other duties as assigned or required.

MINIMUM JOB QUALIFICATIONS:

Education/Certificates/Licenses:

Required

  • Nursing degree from an accredited nursing program
  • Current license in state of practice
  • BLS Certification upon hire
  • Valid driver’s license

Experience:

Preferred

  • Two (2) to five (5) years’ experience preferred in primary care clinical setting
