The RN Certified Diabetes Educator (CDE) at One Community Health (OCH) is responsible for managing the diabetic population across OCH health centers. This role entails providing intensive diabetes management, including medication adjustment, Diabetes Self-Management Education, case management, and behavior change counseling. This position also actively participates in Quality Improvement initiatives related to diabetes management and collaborates closely with PCPCH teams. This position if full-time and based in White Salmon but may require occasional travel to cover other clinic sites.

Qualifications:

  • Associate Degree in Nursing or higher.
  • Background in diabetes patient education and health promotion.
  • Experience in an adult primary care setting.
  • 2 years of general nursing experience.
  • Proficient oral communication skills, with the ability to communicate effectively with patients and coworkers in various settings.
  • Strong teamwork skills combined with the ability to work independently or in a self-directed manner.
  • Exceptional interpersonal, analytical, and problem-solving abilities.
  • Access to own vehicle for off-site work (reimbursable per OCH policy).
  • Proficiency in finger stick blood sugars, EKG, venipuncture, and IVs.
  • Familiarity with patient medications and willingness to seek assistance when needed.
  • Competence in using multi-line telephones and other office equipment.
  • Proficient and accurate keyboarding skills.
  • Strong computer skills, including Microsoft Office applications, Epic EHR, and EPM Systems.
  • Knowledge of and compliance with HIPAA and privacy laws.
  • Ability to work effectively in a diverse, multicultural environment.
  • Capacity to remain calm and effective in high-pressure and emergency situations.
  • Specific knowledge in customer service and workplace safety.

Required Certifications:

  • Registered Nurse (RN) with current Oregon or Washington License, or eligibility to apply for license upon hire and obtain within 6 months.
  • Certified Diabetes Educator (CDE) and/or Board-Certified Advanced Diabetes Management (BC-ADM) certification, or eligibility to take CDE and/or BC-ADM exam once necessary hours are obtained. Must obtain certification within one year from date of hire.
  • BLS/CPR Certified (must be completed within 30 days of hire).
  • Successful completion of OCH credentialing and privilege requirements.

Preferred Qualifications:

  • Fluent bilingual English/Spanish skills.
  • Experience in adult chronic disease management and case management.
  • Proficiency in a variety of educational approaches, including one-on-one instruction and group class settings.
  • Experience with teaching and public speaking.
  • Experience in community and/or migrant health.

Compensation:

  • Hourly/Non-Exempt: $40.00 per hour
  • Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
  • Weekend Differential: This position receives base hourly pay along with an extra $10.00 for each hour worked on weekends (Saturday and Sunday).
