One Community Health is Hiring a Dufur School Based Health Center (SBHC) Provider Near Dufur, OR

The Dufur School Based Health Center (SBHC) Provider will be responsible for delivering direct patient care across all age groups within the scope of credentials and privileges granted within a PCPCH (Patient-Centered Primary Care Home) care model at the Dufur School Based Health Center (SBHC). This includes conducting patient assessments, diagnosing conditions, providing treatment for diagnosed conditions, prescribing medications, patient education, and performing specialized tests and treatments. Additionally, the Dufur SBHC Provider will actively participate in clinical quality improvement, peer review, PCPCH, Meaningful Use, and other programs as required for a Federally Qualified Health Center (FQHC) operating in the State of Oregon.

Minimum Qualifications:

  • Excellent oral and written communication skills, including public speaking.
  • Ability to communicate clinical concepts effectively to both clinical and non-clinical audiences.
  • Proficiency in collecting, researching, analyzing, and presenting statistical data and reports in written form.
  • Capacity to work efficiently within a diverse workforce in a multicultural environment.
  • Demonstrated ability to deliver care to culturally diverse populations with compassion and sensitivity.
  • High-level computer skills, including proficiency in Microsoft Office applications (Word, Excel, Outlook), Electronic Health Record (EHR), and Electronic Practice Management (EPM) Systems.
  • Strong organizational, prioritization, and coordination skills to manage multiple activities/tasks and team members effectively.
  • Capability to delegate tasks and collaborate with team members.

  • Adherence to the standards of nursing ethics and professional conduct as outlined by state statutes and licensing boards in Oregon and Washington.
  • Proven track record of meeting timelines for team and organizational goals with a high level of quality.

Education/Experience:

  • Current Advanced (Family Practice) Nurse Practitioner License in good standing in Oregon and/or Washington.
  • Current DEA certification.
  • Current Basic Life Support (BLS) certification.
  • Strong computer skills.
  • Knowledge and experience in team leadership.
  • Minimum of three (3) years of professional clinical experience in family or general practice, including prenatal, adult and pediatric acute and chronic primary care, office gynecology, and emergency care.
  • Direct experience in performance and quality improvement initiatives, including the Patient-Centered Primary Care Home Model (PCPCH) of care.
  • Maintenance of required Continuing Education (CE) credits.
  • Graduate of an accredited Nurse Practitioner school with current Board Certification in the specialty field.

Preferred Qualifications:

  • Experience in a community or migrant health center (FQHC) setting or public health clinic setting.
  • Experience with practice management systems and Electronic Medical Records (EMR) - EPIC preferred
