One Community Health is Hiring a Director of Outreach Near Hood River, OR

The Director of Outreach holds a pivotal role in orchestrating the planning, execution, and assessment of our diverse health programs designed to enhance health outcomes within our target demographic at One Community Health (OCH). Responsible for fostering a wellness-oriented culture within our community, this role spearheads outreach initiatives to attract existing and potential OCH patients. The Director of Outreach endeavors to deter the onset of chronic diseases in our community and ensure our mission of providing health and social justice to all members of our community.

Qualifications:

  • Fluency in both English and Spanish
  • High School Diploma or equivalent; Bachelor's or Associate's degree in Business Administration or equivalent.
  • Experience with the family-based Patient-Centered Primary Care Home (PCPCH) model of care.
  • 7 years of experience in a supervisory role, preferably in an ambulatory care setting.
  • Current CPR certification.
  • Dedication to enhancing patient health, especially within underserved communities.
  • Demonstrated expertise in overseeing preventative health programs and addressing health disparities.
  • Proficient project management skills with a proven record of successful program implementation and evaluation.
  • Ability to forge partnerships and engage in advocacy endeavors.
  • Familiarity with evidence-based practices and human-centered design principles.
  • Competence in grant writing, fundraising, and managing funding streams.
  • Strong leadership and team-building capabilities.
  • Capacity to lead human-centered design initiatives, prioritizing the patient perspective.
  • Proficiency in developing business cases for new services through revenue and cost analysis.
  • Capability to construct and advocate for team-level budgets.
  • Proficiency in grant application writing.
  • Effective communication skills for team development and positive problem-solving.
  • Knowledgeable in staff management, recruitment, employee relations, training, and workplace safety.
  • Proficient in project management, exercising initiative and sound judgment.
  • Familiarity with medical terminology.
  • Willingness to travel and cover OCH Service Area.
  • Ability to meet deadlines for team and organizational objectives with high-quality outcomes.
  • Accomplished in relationship management within diverse, multicultural teams.
  • Comfortable working in a collaborative environment that values teamwork and adaptability.
  • Ability to inspire trust and communicate effectively with internal and external stakeholders.
  • Capacity to work independently, manage multiple projects, and maintain professionalism and confidentiality.
  • Strong computer skills, including Microsoft Office applications, Epic, and other systems.
  • Knowledgeable about HIPAA regulations, customer service, and workplace safety.
  • Ability to identify learning needs, establish goals, and devise plans to achieve them.
  • Adaptability to a dynamic and evolving community healthcare landscape.

Preferred Qualifications:

  • Certification as a Community Health Worker (CHW).
  • Master's degree in public health, health promotion, or related field.
  • Familiarity with EPIC Electronic Medical Records (EMR).
  • Experience working with low-income, multi-ethnic populations.
  • Background in Community/Migrant Health Federally Qualified Health Center.

Compensation:

  • Salaried/Exempt: $93,000-100,000 annually, DOE
  • Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
