The MDS Coordinator conducts and coordinates the development and completion of the resident assessment in accordance with the requirements of the state and the policies and goals of this facility.
MDS Coordinator Essential Duties and Responsibilities:
·Conducts and coordinates the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of CAAs and Triggers
·Acts as back-up to send denial letters when residents no longer are being skilled for Medicare and falls below Skilled level of Care
·Review all redeterminations requested, prepares for cases to be presented to ALJ from CMA
·Acts as back-up to nursing staff in providing clinical information for BOM to fax updates as needed to insurance companies
·Develops, through independent skills and judgment, implements, and maintains an ongoing quality assurance program for the resident assessment/care plans. Develops action plans to ensure same
·Participates in facility surveys (inspections) made by authorized government agencies, makes effective recommendations on improvements or actions needed
·Obtains the facility and resident QM reports monthly from the state website
·Ensures that all assessments are completed and transmitted in a timely manner. Reports problem areas to the Administrator
·Ensures a current copy of the MDS instructions Manual is available to personnel completing portions of the MDS
·Works with the interdisciplinary Care Plan Team in developing a comprehensive resident assessment and care plan for each resident
·Serves on, participates in, and attends various other committees of the facility (e.g., Policy Advisory, Quality Assessment and Assurance) as required, and as appointed by the Administrator
·Provides written and/or oral reports of the resident assessment/care plan functions as requested or directed by committees
·Participates in functions involving discharge plans as may be necessary
·Evaluates and implements recommendations from established committees as they may pertain to the assessment and/or care plan functions of the facility
·Other duties as assigned

·Must be able to move intermittently throughout the workday
·Must be able to push, pull move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet
·May be necessary to assist in the evacuation of residents during emergency situations
·The noise level in the work environment is usually moderate
