Independent Living (IL) Case Manager

SUMMARY OF WORK PERFORMED
Provide case management services to 25 - 30 youth (case load will vary depending on census). Provide foster youth preparing to age out of foster care transitional support services, including but not limited to, housing, education, employment, basic life skills and permanency. Case managers will be responsible for initial intake through exit processes with youth and will serve eligible youth ages 15-23 years old. Services provided will be in compliance to the IL contract from Washington State DCYF. IL case managers work on behalf of the youth to equip each youth with the skills to successfully transition to independence. **This role will be responsible for serving youth in the greater Clark County area, specifically Vancouver, WA. Must be willing to drive, with mileage reimbursed. Role will be remote to begin with, and will eventually have a "home office" to work from when needed.
JOB DUTIES AND RESPONSIBILITIES:
1. Cultivate contacts and relationships with outside agencies and individuals that have both direct and indirect impact on the youth’s independence and transition. These include but are not limited to:
2. Initiate and complete all documentation including:
3. Meet with youth on a monthly basis as per the contract.
4. Advocate on behalf of the youth with, the Department of Child and Family Services, educational institutions, employment agencies, and other community service providers.
5. Regularly communicate with social workers, foster parents, biological parents and others in the youth’s support system.
6. Develop and maintain a supportive relationship to help guide youth from the foster care system to independence.
7. Participate in regular staffing, team meeting and staff meetings and trainings.
REQUIRED QUALIFICATIONS:
1. Bachelor’s Degree or Master’s Degree from an accredited college or university in social services or similar field.
2. Proficient Microsoft Suite computer skills and access databases.
3. Previous experience working with at-risk children and families.
4. Advanced oral and written communication skills.
5. Strong interpersonal and organizational skills.
6. Valid Washington State driver’s license and acceptable driving record as required by Olive Crest’s insurance carrier.
7. Reliable vehicle and current automobile insurance, indicating your name on the coverage.
8. First Aid/CPR certification for infants and children completed.
9. Must have 2 hours of HIV/blood borne pathogens training in the past 5 years.
10. Criminal record clearance.
11. Ability to lift 25 lbs. and intervene in a physical way during crisis situations with minors up to 18 years of age.
12. Adequate vision and hearing to be able to assess clients needs.
13. Must have a philosophy of support for at-risk children and families in line with the mission, vision, values, and statement of faith of Olive Crest.
PREFERRED QUALIFICATIONS:
1. Preferred Child Welfare experience.
2. Preferred Case Management experience.
SALARY Starting at $23.00 an hour, Full-time 40 hours, Non-exempt
