CASE MANAGER POSITION SUMMARY: As a case manager, you will provide caregiver support services to up to 25 children (case load will vary depending on census). Caregiver support includes the activities that assist the caregiver in navigating the child welfare system to support a child or youth in their care to access needed medical, social, educational, and other necessary services. In partnership with the Olive Crest Licensing team, ensure that each foster and caregiver home is following Olive Crest Foster Family Agency policies, State policies/WACs and COA (Council on Accreditation) standards. Educate, train, and support foster parents in meeting the foster child's overall needs and address any issues that arise.

JOB DUTIES AND RESPONSIBILITIES:

1. Case managers develop relationships and maintain rapport with foster families and caregivers to ensure a supportive partnership and to help guide them through the foster care system.

2. Provide Therapeutic Behavioral Support services for children/youth as assigned. Therapeutic Behavioral Support provides trauma-informed direct supervision to children with high-level needs. These services include:
a. Plan for emergency medical treatment if needed, and ongoing assessment of the child’s health needs.
b. Physical and Behavioral support that ensures health and safety and maintains placement stability.
c. Protective supervision is always in accordance with the supervision plan.
d. Positive Behavioral Support which includes providing a supportive environment and increasing the child’s skill development.

In collaboration with the Supervisor, develop a Caregiver Support Plan for each child placed. This may include attending court hearings, Family Team Decision Making meetings, and general case staffing's on behalf of children assigned to your caseload as directed.

3. Assist with educational advocacy including monitoring progress of school age children, supporting school placement stability, and making recommendations for social and developmental remediation services for preschool age children.

4. Assist with Apple Health to locate providers and schedule appointments.

5. Assist caregivers to identify and access childcare.

6. Provide resources as needed to support the caregiver with parenting skills: in-person, online or in group training.

7. Assist the caregivers as needed to provide culturally relevant services and training.

8. Develop crisis plan that includes after-hours availability for phone and in-person coaching and assistance.

9. Attend to details of crises situations that may arise during working and non-working (on call) hours. Ensure that Serious Incident Reports are written and submitted within 24 hours of the incident.

10. Participate in emergency on-call rotation as needed and when applicable.

11. Initiate and complete all necessary documentation as required by contract and agency.

12. Available to work occasional evenings and weekends as needed for conferences, training and/or responding to children and family crises.

13. Participate in regular group supervision with Care Coordinator Supervisor and regular Program Team meetings.

14. May be assigned a BRS (Behavior Rehabilitation Services) case on an as needed basis.

15. Other Duties as assigned by Supervisor.

REQUIRED QUALIFICATIONS:

1. A bachelor’s degree from an accredited college or university in one of the following: social work; social welfare; marriage, child and family counseling; child psychology; child development; counseling psychology; social psychology; clinical psychology; educational psychology; or education with emphasis on counseling.

2. Previous experience working with at-risk children and families.

3. Proficient Microsoft Suite computer skills.

4. Advanced oral and written communication skills.

5. Strong interpersonal and organizational skills.

6. Valid WA State driver’s license and acceptable driving record as required by Olive Crest’s insurance carrier.

7. Reliable vehicle and current automobile insurance, indicating your name on the coverage.

8. First Aid/CPR certification for infants and children, and 2 hours of HIV/blood borne pathogens training in the past 5 years.

9. Criminal record clearance.

10. Ability to lift 25 lbs. and intervene in a physical way during crisis situations with minors up to 18 years of age.

11. Adequate vision and hearing to be able to assess client’s needs.

12. Must have a philosophy of support for at-risk children and families in line with the mission, vision, values, and statement of faith of Olive Crest.

5. Master’s Degree in related field.
