This is a hybrid position that requires 50% time in clinic and 50% time remotely.

WORK SCHEDULE:

1.0 FTE (40 hours): 11:30am to 8pm for 5 days a week

0.7 FTE (28 hours): 10:30am to 7pm for 3.5 days a week

Building on a legacy established by both organizations in 1972,CommuniCare OLE is a network of federally-qualified health centers with 17sites across Napa, Solano, and Yolo Counties. It offers comprehensive care,including medical, dental, behavioral health and substance use treatment,nutrition, optometry, pharmacy, care coordination, referrals, and enrollmentassistance to more than 70,000 individuals, and no one is turned away due tolack of insurance, immigration status, or ability to pay. Many services areoffered outside of its sites, including mobile health, home visiting, andcommunity and school-based programs.

JOB SUMMARY: The Triage RN is responsible for telephone triage and advice services for OLE Health patients and community partners (such as hospitals, urgent care centers, pharmacies, laboratories, diagnostic imaging centers, and other health care offices). The RN is responsible for answering the nurse triage line, which may include, but is not limited to, performing an assessment of patient symptoms, and referring them to the appropriate level of care (home care, appointment with primary care provider (PCP), and/or escalating treatment to higher level of care like the emergency department). The RN will collaborate with case management, nursing teams, providers, and other departments to ensure quality patient care. The RN will conduct telephone triage following established procedures and protocols, and document calls as required.

EDUCATION, EXPERIENCE, TRAINING

1. Bachelor of Nursing degree from an accredited nursing school preferred.
2. Minimum two (2) years of experience with direct patient care.
3. Current State of California Registered Nurse License required.
4. Must certify and remain current in CPR or BCLS certification.
5. Experience in a community healthcare center preferred.
6. Outpatient primary care setting experience preferred.
7. Training in motivational interviewing preferred.
8. Strong verbal and written communication skills; Bilingual (English/Spanish) language preferred.
9. Strong interpersonal and customer service skills required.
10. Strong computer and data entry skills required; electronic health record system usage required.
11. Strong analytical and problem-solving skills required.
