POSITION TITLE: Behavioral Health Clinician, Navigation Services

REPORTS TO: Behavioral Health Manager, Navigation Center, CalWORKS, Mercy Housing

About CommuniCare OLE

Established in 2023, CommuniCare OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare OLE is a network of federally qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY The Behavioral Health Clinician reports directly to the Behavioral Health manager. Clinician provides individual triage, crisis, assessment, and referral coordination services for those experiencing moderate to severe mental health symptoms and conditions. We strive to provide culturally-responsive, trauma-Informed, and outcome driven care by utilizing an integrated, collaborative, and strength-based approach

EDUCATION, EXPERIENCE, TRAINING

  1. ASW/LCSW, AMFT/LMFT, APCC/LPCC required. Must be in good standing with the Board of Behavioral Science
  2. Applicants must have strong background in mental health assessment, and crisis management.
  3. Bilingual preferred; English/Spanish 
  4. A minimum of two-years of experience working in behavioral health / mental health setting, strong diagnostic and assessment skills preferred.
  5. Direct clinical experience with those living with Severe Mental Illness (SMI) preferred. 
  6. Experience providing services to those experiencing Substance Use Disorders preferred
  7. Drivers License and clean driving record required. 

KNOWLEDGE OF WORK

1.

Completes ASAM screening, progress notes, assessments, and QA audits in a timely manner and according to Drug Medi-Cal guidelines.

2.

Provides information to clients including treatment alternatives and referral service.

4.

Knowledge of Trauma-Informed principles and practice

 DUTIES AND RESPONSIBILITIES

1.

Clinician is a member of the Navigation Center team and is responsible for: responding to crisis, triaging client needs and conducting comprehensive biopsychosocial assessments for clients experiencing moderate to severe mental health symptoms and conditions, care coordination and referral, understanding billing structure, and how to correctly utilize and interpret ICD10 and DSM5 codes and criteria.

2.

Collaborate directly with program case managers, peer partners, supervisor, community stakeholders, and the Yolo County Department of Health and Human Services to coordinate long term follow-up and care.

3.

Complete clinical documentation within 48 hours of services in accordance with working hours and agency, county, and contract requirements.

4.

Provide culturally responsive counseling, assessment, and activity services for diverse populations with a wide range of needs/acuity, including Substance Use Disorders

5.

Monitor patient charts to ensure the timely placement of all correspondence, completes all screening and data collection tools, tracks caseload, and completes discharges according to protocol and best practices. Participate in quality assurance and improvement efforts.

6.

Manage complex caseload, direct service, and administrative duties in fast-paced setting.

7.

Respond to and manage crisis situations according to agency guidelines and legal/ethical mandates.

8.

Work with clients to establish short-term goals and safety plan. Clinician is responsible for detecting emotional, psychological, and physical risk factors and danger signals. Clinician is non-judgmental and offers feedback and support to clients.

9.

Provides information to clients including treatment alternatives, referrals, and other community resources.

10.

Responsible for remaining within the legal/ethical standards, including confidentiality and HIPAA, mandated reporting, and Tarasoff.

11.

Responsible for understanding, explaining and enforcing all rules and regulations associated with clinic policy.

12.

Compile and report on outcome data as required.

13.

Attend clinical supervision/consultation, weekly staff meetings, meetings with community partners, training as arranged/required.

14.

Maintains current license/registration in good standing, submits all CEU and training certifications to HR.

15.

Responsible for maintaining the standards as listed in the CommuniCare OLE’s Policies and Procedures, and any other standards which may apply to this position.

16.

May require direct transportation (in personal vehicle) of clients as part of navigating services and appointments. 

17.

Other duties as assigned.
