Established in 2023, CommuniCare OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY:
BH Clinician in School Services provides mental health and supportive services to students and families in Woodland Joint Unified School District. Services include individual and group psychotherapy, immersion, and collaboration with various school sites.


EDUCATION, EXPERIENCE, TRAINING

  • ASW/LCSW, AMFT/LMFT, APCC/LPCC required. Must be in good standing with the Board of Behavioral Science
  • 2 years minimum working in the behavioral health/mental health field
  • Bilingual preferred; English/Spanish

SECTION I: PERFORMANCE ACCOUNTABILITY


  • KNOWLEDGE OF WORK

1.

Effectively utilize measurement-based care practices to determine client progress, wellness and risk factors; collect required departmental pre/mid/post health screenings (e.g., PHQA, CANS).

2.

Ability to address a wide range of mental health conditions, utilize a variety of assessment tools, facilitate diagnostic evaluations and treatment interventions to address client needs, goals, and circumstances.

3.

Competently deliver therapy services to students and families with client goals being at the center of care.


B. DUTIES AND RESPONSIBILITIES

1.

Provide in-person and telehealth services including assessment, triage, crisis support, and individual and/or group therapy for students experiencing a range of mental health symptoms in a trauma-informed and culturally responsive manner.

2.

Collaborate with school staff, WJUSD, and Yolo County Health and Human Services Mental Health to provide and coordinate care, including substance use disorders and level of care changes.

3.

Independently manage complex caseload at various school locations throughout WJUSD which includes coordination with staff, travel between school sites, and service and support to students and their families

4.

Complete clinical documentation within 48 hours of services in accordance with working hours, all documentation must comply with Medi-Cal, Beacon Health guidelines. Participate in peer chart reviews and other quality improvement efforts.

5.

Available for in-person and video-based crisis, triage, and consultation during and after school hours.

6.

Complies with established department policies and procedures, objectives, quality assurance program, safety, environmental, and patient service standards.

7.

Based on the individualized student assessment, develops a treatment plan of provision of services in collaboration with the client and includes measurable goals.

8.

Identifies referral needs. If needed, makes referrals for more intensive psychological or psychiatric services.

9.

Refers students with apparent need for psychotropic medication to primary care provider and/or psychiatrist where appropriate.

10.

Participates in integrated system of care, staff meetings, case review, consultation/supervision sessions and other forums as required.

11.

Conducts all work within generally accepted standards and ethical guidelines of the social work or counseling professions. Seeks out guidance from School Services Supervisor/Youth and Family Services Manager when questions or concerns arise.

12.

As safe and appropriate to do so, transport clients or support with transportation arrangements.

13.

Creates a supportive discharge process, in collaboration with student and school/district staff.

14.

Other duties as assigned.
