About CommuniCare OLE

Established in 2023, CommuniCare OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare OLE is a network of federally qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

The following statements are intended to describe the major elements and requirements of the position and should not be taken as an all-inclusive list of responsibilities, duties, and skills required of individuals assigned to this job.

JOB SUMMARY The Behavioral Health Clinician (AMFT/LMFT, ASW/LCSW, APCC/LPCC) provides adult mental health and/or substance use disorder assessment and counseling services to clients who have been referred for justice system diversion. Clinician supports low-barrier access to services and affirms the client as the expert in their own life. Services are facilitated in clinic and community settings. Clinician offers ideas and solutions to program challenges and supports other team members to do their best work. Demonstrates cultural humility and is passionate about serving those who have justice system involvement, a history of incarceration, trauma, substance use disorders, severe mental health conditions, and chronic homelessness. Collaborates with other clinical, case manager, peer, and supervisory staff by using a strength-based and developmental approach where learning is key. The Clinician is open to feedback and shares work-related information with peers and supervisors as appropriate and necessary. Clinician understands the role of stigma, discrimination, and systemic factors that create barriers to stability and wellness. All services are provided in-person (in rare instances, telehealth may be used with approval)..

EDUCATION, EXPERIENCE, TRAINING

  • ASW/LCSW, AMFT/LMFT, APCC/LPCC required or application in process. Must be in good standing with the Board of Behavioral Science
  • 2 years’ experience working in a behavioral health/mental health/SUD treatment setting is preferred.
  • Experience conducting biopsychosocial assessments, treatment planning, and therapy is required.
  • Direct clinical experience working with those living with co-occurring conditions required.
  • Experience or knowledge of re-entry services, diversion programs, collaborative courts, restorative justice, and/or harm reduction strongly preferred.
  • Bilingual English/Spanish preferred.

SECTION I: PERFORMANCE ACCOUNTABILITY

  • KNOWLEDGE OF WORK
1.
Knowledge of DSM 5 and ICD-10 codes.
2.
Clinician is responsible for: responding to crisis, triaging client needs, conducting comprehensive biopsychosocial assessments for clients experiencing moderate to severe SUD/MH conditions, diagnosing, treatment planning, providing therapy, care coordination and referral, understanding billing structure, and how to correctly utilize and interpret ASAM, ICD10 and DSM5 codes and criteria.
  • DUTIES AND RESPONSIBILITIES
1.
Collaborate with program case managers, peer advocates, supervisor, community stakeholders, and the Yolo County Department of Health and Human Services to coordinate long term follow-up and care.
2.
Adapts clinical approach and interventions to support harm reduction or abstinence-based recovery as indicated by supervisor and/or clinical staff and/or referral.
3.
Complete clinical documentation within 48-hours of services in accordance with working hours and agency, county, and contract requirements.
4.
Ensures clients have access to care by meeting or exceeding minimum direct service requirements (minimum of 65% of working hours must be engaging in face-to-face clinical services).
5.
Demonstrate cultural humility and respect when working with clients, peers, and community partners.
6.
Monitor client charts to ensure the timely placement of all correspondence, complete all screening and data collection tools, track caseload. Participate in quality assurance and improvement efforts.
7.
Manage complex caseload, direct service, and administrative duties in fast-paced setting.
8.
Work with clients to establish goals and treatment/care plan. The clinician is responsible for detecting emotional, psychological, and physical risk factors and danger signals.
9.
Creates a supportive discharge or program completion process with other team members.
10.
Responsible for remaining within all legal/ethical standards, including confidentiality and HIPAA, mandated reporting, and Tarasoff.
11.
Responsible for understanding, explaining, and enforcing all rules and regulations associated with clinic and program policy.
12.
Compile and report on outcome data as required.
13.
Attend clinical supervision/consultation, weekly staff meetings, meetings with community partners, training as arranged/required.
14.
Maintains current license/registration in good standing, submits all CEU and training certifications to Human Resources.
15.
Responsible for maintaining the standards as listed in the CommuniCare OLE’s Policies and Procedures, and any other standards which may apply to this position.
16.
With supervisor approval and as safe to do so, transporting clients in own vehicle could be required if program vehicles are not available (mileage reimbursed).
17.
Other duties as assigned.
