Old Colony Elder Services is Hiring a Housing Specialist Near Brockton, MA

OUR HOUSING TEAM IS GROWING! Be a part of this growth and help make a real difference in the lives of our consumers.

OCES is seeking a dedicated, caring professional who enjoys working in the community to work with our Housing team to ensure services and support to residents on-site in our new Brockton area locations.

What you will do:

  • Review the needs of residents in order to link to community programs and services.
  • Serve as the Multi-disciplinary Assessment Team Leader and meet with the team to assure service coordination.
  • Facilitate care conferences with residents, family, and other participants.
  • Plan and coordinate social, educational, and recreational programs for residents.
  • Prepare and distribute a newsletter to keep residents informed of planned activities.

What you'll bring:

  • Your passion for helping others and desire to make a difference
  • Adaptability to be able to change as the needs of consumers and/or programs change
  • At least two years of experience in the human services field preferably with the elder population
  • Bachelor's degree in Social Work or related field preferred
  • Bilingual; Haitian Creole or Cape Verde Creole is required
