Job Summary:
Contact patients discharged from in-patient hospital settings for Transition of Care appointments. Assess and support treatment regimens and adherence to medication management. Assist the patient/patient’s family in obtaining access to medical care as needed. Obtaining various lab tests/cultures to the standards of care protocols as ordered by the physician. Obtain and review hospital discharge information needed for follow-up. Direct contact for assigned population health patient panels and assist with their triage needs. Prior Authorizations and home Health Care Processing. Perform Population Management functions for Family Medicine patients. Monitor the quality and effectiveness of interventions to the population by setting long-term and short-term specific, measurable goals. Accessing and systematically using data from multiple sources such as medical records, claims data, MIPS data, and other program metrics to target specific populations for outreach, education, and intervention. Facilitate quality improvement activities that educate, support, and monitor patients' population health management. Perform other duties as assigned by Practice Administrator, Nursing Supervisor, and Family Medicine Department Chair.

Possible exposure to hazardous material, jot and cold, blood borne pathogens.
Physical Requirements:
Ability to lift and carry 20 pounds, stoop, reach, stand, walk, finger, grasp, feel, talk, hear, see, and perform repetitive motions with or without reasonable accommodations.
