Title: Case Manager
* This is a full-time HYBRID position covering the SE counties in Ohio to include, but not limited to, Hocking, Perry, Athens, Vinton, etc.
* Monday-Friday with hours of 8:00am-5:00pm that may include, on-call rotations days, evenings, nights, weekends and holidays.
The Pure Healthcare Case Manager will include remote work as well as making occasional in home visits. You will develop comprehensive person-centered care plans for every member, with a focus on assuring timely and appropriate access to care and the skillful coordination of broad array of medical and social services. This position affords the opportunity to work with individuals, providers and multidisciplinary team members to assess, facilitate, plan and coordinate an integrated delivery of care from acute care across the continuum. The Case Manager is the key to ensuring a collaborative and proactive approach to support the individual and caregiver in the management chronic medical conditions.
Case Manager Qualifications:
- Active Registered Nurse license or Active Licensed Social Worker in the State of Ohio without any board actions.
- Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred; or Bachelor’s degree in social work from a program accredited by the Council on Social Work Education
- One to three years clinical experience in Case Management, Care Coordination, Discharge Planning, Medicaid and/or Medicare managed care, LTC experience and/or community health. Mental Health Experience is a plus.
- CM certification preferred, but could be obtained after two years of employment.
- CPR Certified required.
- Computer skills sufficient to properly document services and care.
- Ability to drive during daytime, nighttime, or inclement weather. This position requires travel to different work locations and within the counties we serve.
- Valid Driver's License with Safe Driving Record with successful clearance of a driving record check
- Successful clearance of a criminal background check
- State minimum automobile insurance coverage.
