We are looking for a Community Health Worker (Promotor de Salud) to join our team and help us provide quality healthcare services to our community. The ideal candidate will have a passion for helping others and a strong understanding of the social determinants of health. The successful candidate will be responsible for providing professional medical interpretation, culturally-responsive health education, connecting individuals to resources, and advocating for policy changes that benefit the community. This is an excellent opportunity for someone who is looking to make a difference in their community.

Responsibilities:

  • Provide clinical support to patients in federally-qualified health clinics
  • Use their language skills to inform, educate, and interpret for Limited English Proficient patients
  • Provide health education and resources to the community.
  • Assisting patients in understanding medication and treatments
  • Assist in the patient's daily operations of the clinics by answering phones, calling patients, setting appointment reviewing and or filing medical charts.
  • Maintaining patient's confidentiality
  • In compliance with HIPPA (Health Insurance Portability and Accountability Act of 1996)
  • In compliance with Patients' Rights
  • Work with clinical staff in a professional matter and insure patient’s safety
  • Communicate all patients concerns effectually and clearly with health providers
  • Understands and is willing to travel around Central Ohio areas and/or be assigned to other clinics when needed.
  • Communicate effectively and timely any problems, concerns, or comments surrounding patients or staff members to supervisors.
  • Assist with health screenings and assessments.
  • Refer individuals to appropriate healthcare providers.
  • Provide support to individuals and families in navigating the healthcare system.
  • Build relationships with community members and organizations.
  • Maintain accurate records of services provided.
  • Assist with outreach activities.
  • Other duties as needed.

Qualifications:

  • Fluent English and Spanish Language Proficiency
  • HS Diploma or equivalent.
  • Valid Driver’s License and reliable vehicle.
  • Willingness and flexibility to travel around Central Ohio area.
  • Legally authorized to work in the U.S.
  • Must be at least 18 years of age.

Job Types: Part-time, Contract

Pay: $15.00 per hour

Expected hours: No more than 40 per week
