Registered Nurse - Home Health
Duties:
- Provide skilled nursing care to patients in their homes
- Assess patient needs and develop individualized care plans
- Administer medications and treatments as prescribed by the physician
- Monitor patient vital signs and response to treatment
- Educate patients and their families on proper healthcare techniques and disease management
- Coordinate with other healthcare professionals to ensure comprehensive care for patients
- Document patient assessments, interventions, and outcomes accurately and timely
Skills:
- Strong knowledge of human physiology and anatomy
- Proficient in ostomy care and management
- Experience with wound care
- Ability to assess and prioritize patient needs
- Knowledge of OASIS standards and scoring
- Familiarity with creation and management of plan of care
- Competent in overseeing nursing care
- Skilled in administering injections and IV safely and accurately
- Understanding of safe discharge planning
- Proficiency in home health charting
Requirements:
- Valid Registered Nurse (RN) license in the state of California
- Minimum 3 years of experience as a Registered Nurse, preferably in home health or a related field
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Strong critical thinking and problem-solving abilities
- Flexibility to adapt to changing patient needs and schedules
