Department:Nursing

Salary Grade: Exempt Status Monday-Friday 7:30 am- 3:30 pm 

Supervises: Charge Nurses and Nursing Staff

This job description, defining qualifications, primary (essential) and secondary (non-essential) functions, responsibilities and duties, is intended as a guideline for the employee and manager.The duties listed here are not considered to be all inclusive.The employee in this position performs these and other duties as required, although not listed, which may be assigned by his supervisor or facility Administrator/designee.

ESSENTIAL FUNCTIONS:

Management Function Requirements:

Under the direction of the Director of Nursing Services, holds 24-hour authority, responsibility, and accountability for the clinical management of assigned resident/patient care unit.

Monitors compliance with established budgetary and reimbursement guidelines.

Assumes the essential responsibilities of the Director of Nursing in her absence.

Conducts regularly scheduled staff meetings.

Participates in regularly scheduled nursing department meetings.

May participate or coordinate in multi-disciplinary meetings.

Individualizes nursing orientation to meet orientee’s needs in collaboration with DON and SDC.

Periodically makes unit rounds and reports to appropriate Department Head as necessary.

Makes resident/patient rounds with a report to the Director of Nursing Service.

Directs, evaluates and supervises all resident/patient care and initiates corrective action as necessary.

Conducts quality assurance and continuous quality improvement monitoring.

Assesses resident equipment maintenance and supply needs, and makes recommendations to the Director of Nursing Service.

Completes reports and assignments accurately and adheres to established time schedules.

Establishes open lines of communication between all nursing personnel and consistently takes follow-up action as necessary.

Counsels and disciplines professional and non-professional nursing personnel and recommends personnel action to the Director of Nursing Service as necessary.

Observes infection control policies and procedures relating to nursing services.

Provides for effective distribution and utilization of personnel on nursing units.Evaluates staff assignments and assists staff in planning.

Directs and supervises activities of professional and non-professional nursing personnel

Oversees monthly editing of physicians’ orders, medication, treatment and behavior sheets

Coordinates services rendered by nursing personnel with those rendered by personnel of other departments.

Supports and participates in Inservice Education Programs.Provides instruction and demonstration to staff on units when needed.

Evaluates staff performance.

Encourages personnel to further their educational growth and development.

Monitors infection control practices of nursing personnel including correct implementation of Universal Precautions and oversees reporting of infections to Infection Control Coordinator.

Completes Infection Control Log in the absence of the Director of Infection Control.

Ensures that physicians are notified of changes in residents' conditions, weight, medication errors, medication not available, abnormal laboratory/X-ray results and any other unusual observations.

Resident/Patient Care Requirements:

Ensures confidentiality of all resident/patient and employee issues.

Reviews residents/patients admitted and compares physician orders with each resident's/patient’s plan of care to assure appropriate implementation of the physician's plan of care.

Maintains, monitors and assists licensed personnel with special services including but not limited to:

a.Central Venous Access Devices and TPN therapy.

b.Capillary blood glucose monitoring.

c.Respiratory Care

d.Intravenous therapy

e.Coma Care

f.Enteral feedings

Administers medications and treatments as necessary.

Documents resident/patient care provided and resident's/patient’s response or lack of response to care provided as necessary.

Reports changes in residents'/patient’s conditions to a physician, Director of Nursing Service and responsible party promptly and takes follow-up action as necessary.

Follows facility admission procedures and communicates resident/patient admission to other departments, physician and ancillary service providers.

Knowledgeably and competently performs basic nursing skills.

Monitors facility admission, discharge/transfer procedures and assures appropriate coordination with other providers.

Assesses residents'/patients’ needs and initiates development of individualized plans of resident/patient care.

May coordinate resident/patient care conferences, as necessary.

Provides direct resident/patient care and gives instruction to nursing assistants related to resident/patient care, as necessary.

Listens to residents'/patients’, families, or physicians' concerns and initiates corrective action and/or reports concerns to Director of Nursing

May perform RN pronouncement of death according to facility policy and procedures.

Safety & Health Requirements:

Identifies safety hazards and initiates corrective action as necessary.

Monitors the safety of residents/patients under his/her supervision.

Observes all facility fire, safety and evacuation policies and procedures.

Monitors and maintains the physical environment for cleanliness, safety and security.

Identifies resident/patient problems and emergency situations and initiates immediate "life saving" measures within the scope of nursing practice and in accordance with advance directives.

 Professional Development Requirements:

Participates in committees and meetings as assigned by the Director of Nursing.

Strives for continuous self-improvement and institutes continued education, formal and self-directed.

Comes to work clean and neat; and consistently presents a professional appearance in accordance with facility dress code

Accepts assigned duties in a cooperative manner.

Documentation Requirements:

Monitors and/or completes resident MDS assessmentsand care plans for timely and accurate completion and follow-up action as necessary.

Edits monthly physician's orders, medication, treatment and behavior sheets, as necessary.

Reviews and evaluates nursing documentation including nursing summaries, nursing notes, medication, treatment and behavior sheets and nursing assistant flow sheets when necessary.

Reviews the clinical record for accuracy and completeness periodically and reports to the appropriate Department Head.

General Requirements:

Demonstrates knowledge and understanding of the impact of disability upon an individual's, as well as families' physical, emotional, social, economic, and vocational status, taking into consideration age-related issues, and developmental levels over the life span (i.e., the adolescents' need to develop their own identity and cope with life changes; and the geriatrics' need to maintain their integrity and independence and adjust to the physiological aspects of aging), and meets competency requirements for the population served (adolescent through geriatric), as applicable to the job description.

Follows facility procedures related to control of equipment and supplies within the unit.Assures that work assignment areas are clean and equipment and supplies are stored appropriately.

Consistently works cooperatively with administration, all facility personnel, physicians, community agencies, residents/patients, families and consultants.

Interprets existing policies and procedures to professional and non-professional nursing personnel.

Follows established standards of nursing practice and implements facility policies and procedures.

Regularly reviews service on the unit for compliance with all State, Federal and JCAHO standards and regulations and immediately reports all irregularities to the Director of Nursing.
