Ocean Gardens Care Center is Hiring an ASSISTANT DIRECTOR OF NURSING Near Queens, NY

POSITION SUMMARY: - Assists with the planning, organization, development, coordination and direction of the Nursing Department in its Programs and activities in accordance with current Federal, State and Local guidelines and regulations, accepted standards of practice and as may be directed by Administration and/or Director of Nursing.

REPORTS TO:

• Director of Nursing

JOB FUNCTIONS:

QUALIFICATIONS:

  • Registered Nurse with a current NYS license.
  • Nursing degree from an accredited college or university. Graduation from an approved school of Nursing and experience may be considered in lieu of a degree.
  • Previous experience in a long term care or subacute facility. 
  • Prior Nursing administrative/supervisory experience in a long-term setting.
  • Knowledge of MDS and care planning.
  • Knowledge of regulatory requirements governing long term care facilities.
  • IV certification a plus.

THE FOLLOWING DUTIES/RESPONSIBILITIES ARE ESSENTIAL FUNCTIONS OF THE JOB THAT MUST BE SATISFACTORILY MET WITH OR WITHOUT ACCOMMODATION.

DEPARTMENTAL:

  • Assists with the planning, organization, development, coordination and direction of the Nursing Department in its Programs and activities in accordance with current Federal, State and Local guidelines and regulations, accepted standards of practice and as may be directed by Administration and/or Director of Nursing.
  • Directly supervises assigned staff which includes but is not limited to, participating in orientation, manage staff performance in relation to the position’s standards, and monitor time and attendance.
  • Exercises good independent judgment and discretion in effectively managing assigned staff by, but not limited to, conducting performance appraisals, corrective disciplinary action and other supervisory interventions which may affect a change in the assigned staff’s status of employment.
  • Makes recommendations to the Director of Nursing, as to the hiring, firing, advancement, promotion or any other change in employment status.
  • Carries out major assignments in conducting the clinical care operations of the business.
  • Has authority to negotiate and bind the facility on significant matters.
  • Investigates and resolves matters of significance on behalf of management.
  • Represents the facility in handling complaints, arbitrating disputes, or resolves grievances.
  • Nursing responsibilities have a significant impact on the clinical care operations of the facility.
  • Has the authority to formulate, affect, interpret, and implement management policies and operating practices with regard to Nursing responsibilities.
  • Responsibilities affect facility operations to a substantial degree.
  • Plan, develop, organize, implement, evaluate, and direct the interdisciplinary team, its programs and activities. 
  • Coordinate the team staff to assure the continuity of the resident’s plan of care and day to day operation of the service. 
  • May participate in the development and maintenance of facilities policies and procedures. 
  • Evaluates the needs of the specific service and devises staffing requirements which will accomplish the stated objectives and standards of the service/facility and submit recommendations to the Director of Nursing.
  • Perform evaluations for each level of team members or delegate others to do so and assure compliance on a timely basis.
  • Interprets the facilities policies and procedures to employees, residents, visitors, and government agencies. 
  • Develop and participate in the planning, conducting, scheduling, etc. of inservice training classes (Staff Development), on the job training and orientation programs for nursing service personnel, as well as for support personnel involved in providing care/services to the resident.
  • Perform administrative duties such as completing necessary medical forms/reports, evaluations, studies, charting, etc.
  • Participate in the recruitment and selection of personnel for the team.
  • Delegate administrative authority, responsibility, and accountability to nursing personnel and team members as deemed necessary to perform their assigned duties.
  • Directs the Nurse Managers to orient, instruct, and supervise personnel, and to identify staff’s needs for further inservice education and training programs. 
  • Review complaints and grievances and submit written as well as oral reports to the Director of Nursing.
  • Keep abreast of economic conditions/situations of the specific service and recommend adjustment in the service that ensures the continued ability to provide quality care.
  • Recommend to the Director of Nursing the equipment and supply needs of the department. 
  • Assure that nursing personnel/team follow established safety regulations in the use of equipment and supplies, providing care/services, etc., at all times. 
  • Assure that all employees, residents, and visitors followed the established facilities policies/ procedures.
  • Supervise Wound Care/Dermal Ulcer Care Program and maintain a record of it.
  • Conducts weekly Wound Care/Dermal Ulcer Round with the interdisciplinary team.
  • Provides for the direction of the assigned service on a 24 hour basis. 
  • Assures implementation of resident’s rights. 
  • Ensures that staff is aware of the physical, rehabilitation, nutritional, emotional and spiritual needs of the resident. 
  • Works closely with the Director of Admissions to ensure that each prospective admissions health care needs can be met by reviewing PRI’s and other medical information. 
  • Evaluates the performance of the Inservice Coordinator, MDS, Medicare Coordinators, Nurse Managers as appropriate. 
  • Assures staff compliance with established policies and procedures and standards of practice and enacts procedural steps of correction and discipline, as necessary. 
  • Coordinates Performance Improvement activities including assisting in gathering data, analyzing and initiating/presenting recommendations to the Director of Nursing.
  • Investigates and reports any suspected or incidents of resident abuse and report such to Department of Health if indicated.
  • Participate in Discharge Planning.
  • Maintain the confidentiality of all resident information as per HIPAA guidelines.
  • Keeps Administration/Nursing Administration advised of potential/actual safety hazards.
  • Identifies educational needs of Nursing staff and provides individual, group and unit education/inservice.
  • Coordinates, participates in all resident investigations, evaluation of accident/incident. Initiates investigations when appropriate.
  • Ensures all accident/incidents are completed to facility’s policies and procedures. Reviews all accidents/incidents with Director of Nursing and Medical Director for implementation of corrective actions.
  • Monitor accidents related to medical equipment/devices.
  • Evaluates use of fall prevention. i.e. floor mats, bolster, bed alert, chair alarm, etc.
  • Compiles statistics, establishes patterns, trends for review at the Safety Committee Meetings.
  • Participates in the interdisciplinary review of all accidents/incidents.
  • Notifies Director of Nursing, Administrator and report to the New York State Department of Health accident/incident when necessary according to State and Federal guidelines.
  • Ensures that resident next of kin or contact person in record is notified of the accident/incident.
  • Performs as “Administrator on Duty (AOD)” as assigned.
  • Performs other duties as assigned. 

FACILITY-WIDE:

  • Must possess the ability to consistently come to work as assigned, including coming to work regularly and on time.
  • Must possess the ability to adhere to the time recording policy, including adhering to meal/break policy/schedules, as assigned.
  • Must comply with departmental dress code requirements including prominently wearing one’s ID card, and maintaining normal levels of personal hygiene and professional appearance.
  • Must have the ability to satisfactorily meet work performance and productivity standards as they relate to the position.
  • Must participate in and adhere to facility programs and activities such as but not limited to, employee health, safety, security and corporate compliance.
  • Must participate in facility mandatory in-service and job training programs and activities, as assigned.
  • Must perform all duties and all interactions in accordance with current Federal, State, Local guidelines and regulations, Facility policies and procedures, and accepted standards of practice.
  • Must have the ability to uphold the highest standards of ethics, integrity and honesty in matters of conduct and behavior as it pertains to work performance, position and profession.
  • Must have the ability to effectively and professionally cope with and manage interactions with patients/residents and their family members and/or visitors, as well as, co-workers, to ensure the best possible care, customer service and satisfaction.
  • Must have the ability to maintain confidentiality in all matters concerning Horizon Care Center resident/patients as well as assigned computer/software and work product.
  • In an emergency, may be asked to perform other functions as deemed necessary. 
  • Maintain the confidentiality of all residents’ information as per HIPAA guidelines.

  • Must maintain and timely provide facility with current license/certification as required by the position.
  • Must be able to read, write, speak and understand the English language with sufficient proficiency to meet the essential functions of the position.

  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing Nursing Department practices.
  • Must demonstrate ability to maintain satisfactory competency levels and professional standards of practice.
  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately to assure that the essential functions of this position can be fully met.
  • Must have the ability to operate a computer or use prosthetics or devices that will enable such use.
  • Must be physically able to operate Horizon Care Center specific equipment in relation to the position or use prosthetics or devices that will enable such use.

SETTING:

  • Must be able to perform duties throughout Horizon Care Center.
  • Must be able to sit, stand, walk, climb, stretch, and bend with sufficient ability to meet the essential functions of the position.
  • Must be able to stand, lift, push and pull at least 50 lbs
  • Must be in good general health and demonstrate emotional stability with sufficient proficiency to meet the essential functions of the position.
  • May be subject to frequent interruptions
  • May be subject to hostile and emotionally upset residents, family members, etc. 
  • Must be able to work beyond normal working hours, on weekends, and in other positions temporarily, when necessary. 
  • s subject to stay or call back during an emergency (such as but not limited to severe weather, utilities failure, evacuation, disasters etc)
