Oaks Integrated Care is Hiring a Case Manager Near Cherry Hill, NJ

Oaks Integrated Care is actively seeking qualified Case Managers for our Jail diversion and correctional case management program located in Camden County, NJ. The program works closely with Burlington County law enforcement to divert nonviolent mental health consumers from the jail system to appropriate mental health services in the community.

The Case Manager will have the ability assess and interpret information related to population; have knowledge of the symptoms of the disability/disorder and ability to recognize exacerbation of symptoms; have a general understanding of treatment needs; knowledge of available treatment resources and appropriate use; Population-specific communication skills.

Driving is required for this role.

Responsibilities:

  • Provide intensive community-based services to seriously and persistently mentally ill consumers in the community
  • Facilitate service linkage in the mental health and non-mental health, health system; coordinating and integrating services from multiple providers
  • Provide direct service support including: transportation, coordinating treatment team meeting; filing applications for services; aggressive advocacy; escort to appointment; daily living skill remediation and money management
  • Maintain a consumer clinical record (electronically) that accurately reflects all services provided on a timely basis and meets Agency and funding source standards
  • Complete Daily Activities in the Electronic Clinical Record
  • Provide 24 hours/7 days a week on call crisis intervention on a rotating basis, including evenings and weekends
  • Work collaboratively with the Clinical Team
  • Drive and travel within Camden County

Qualifications:

  • Masters of Social Work (MSW) or a Master’s (MA/MS) degree with clinical training (MSW must have LSW) OR;
  • Bachelor’s (BA/BS) degree in the mental health field (Social Work, Psychology, etc.) plus (1) year post-degree relevant experience;
  • Valid drivers license in good standing required.

*Responsible for clearances pre and post-hire in accordance with Oaks Integrated Care and County Jail System*

All positions required a valid driver's license in good standing
