Job Description

Customized Assistance Services (CAS) helps Human Resources Administration (HRA) clients with health and/or mental health conditions reach their highest attainable level of functioning and self-sufficiency by providing comprehensive, integrated, individualized clinical and support services. CAS works with other components of HRA and with other governmental and non-governmental service providers to create new programs and to integrate and refine existing services so the people it serves can achieve their maximum functional capacity.
Under the general supervision of the We-CARE Operations Deputy Director with latitude for independent judgment, decision-making and action, the We-CARE Program Support Specialist functions as an out-stationed liaison at We-CARE vendor locations. Representing the Human Resource Administration (HRA) and utilizing clinical expertise, the We-CARE Program Support Specialist provides social work support services at either the primary We-CARE vendor or subcontractor(s) location, facilitate the exchange of information between Family Independence Administration (FIA) and vendor staff, and provide clinical and technical assistance on We-CARE procedures and HRA employment policies and procedures.
The Customized Assistance Services, Office of Rehabilitation Services We-Care Operations (ORS) is recruiting for one (1) Consultant Public Health (SW), to function as a We-Care Program Support Specialist, who will:
  • Provide service interventions for We-CARE clients and clinical advice to We-CARE vendor/subcontractor staff on the provision of services to clients with limitations to employment requiring Vocational Rehabilitation Services, participate in a Wellness Plan or pending Federal Disability benefits.
  • Review and evaluate clinical information and provide recommendations to We-CARE Operations Deputy Director for improving service delivery to We-CARE clients at vendor/subcontractor sites.
  • In conjunction with We-CARE Operations Deputy Director, develop and complete reviews of service gaps in meeting the needs of the We-CARE program participants and formulate program improvement recommendations.
  • Provide liaison services, linking vendor/subcontractor staff with HRA departments; provide referral and follow up services for clients with special circumstances.
  • Provide clinical workflow to the vendor’s case management staff and ensure that client status plans are updated in a timely and appropriate manner and maintained in HRA’s case record.


Minimum Qualifications

Graduation from an accredited graduate school of social work as evidenced by a master's degree or certificate and four years of full-time paid experience in public health, medical or psychiatric social work, at least two years of which must have been in a supervisory, administrative or consultative capacity.
Within 18 months of the date of appointment, all candidates will be required to obtain a certificate as Certified Social Worker (CSW) issued by the New York State Department of Education.


Preferred Skills

- Valid New York State Social Worker License - Excellent verbal, written and interpersonal communication skills

Residency Requirement

New York City residency is generally required within 90 days of appointment. However, City Employees in certain titles who have worked for the City for 2 continuous years may also be eligible to reside in Nassau, Suffolk, Putnam, Westchester, Rockland, or Orange County. To determine if the residency requirement applies to you, please discuss with the agency representative at the time of interview.
