Position Description

A significant function of the Social Work Reentry Department includes identifying and supporting individuals eligible for appropriate reentry services including Assisted Outpatient Treatment (AOT) and the Office for People with Developmental Disabilities (OPWDD). The AOT and OPWDD Coordinator will support patients by working closely with their assigned treatment team and help to coordinate services, including but not limited to: assessing for and submitting appropriate referrals, and, facilitating case conferences. This full-time position reports to the Director of Social Work, through the Clinical Coordinator of Social Work, and is responsible for providing coordination between CHS and either the DOHMH’s AOT Rikers team and / or the NYS OPWDD, with the purpose of ensuring that the patients with some of the more complex reentry needs leave NYC jail with the appropriate follow up and supervision.

The AOT population includes SMI Patients with history of non-compliance with treatment leading to significant risk to self or others. The OPWDD population includes those with neurodevelopmental disability as evidenced through IQ and Adaptive Functioning evaluations.

Detailed Tasks

Serving as a liaison between the NYC Health Hospitals CHS Social Work Reentry Department and the NYC Department of Health and Mental Hygiene (DOHMH), the NYS Office for People with Developmental Disabilities (OPWDD), as well as transitional case management agencies, community treatment and housing providers, and other agencies serving these populations.

· Organizing, participating, and/or leading regular meetings with City and State agencies.

· Providing education to department staff about the eligibility and overall service provision by AOT and OPWDD.

· Screening AOT & OPWDD referrals for completeness and appropriateness and overseeing submissions and tracking status of pending referrals, providing/obtaining updates as needed.

· Tracking and reporting on clients actively under AOT orders or with OPWDD eligibility, including facilitation of case conferences.

· Assisting CHS social workers and case managers to develop clinically informed and individualized discharge plans and identify and utilize community resources for Care Coordination, case management, mental health, substance use, entitlement benefits, housing, and medical care for individuals on the caseload.

· Promoting and supporting the Social Work Reentry Department collaboration with an interdisciplinary treatment team approach to patient care through excellent verbal and written communication skills.

· Facilitating communication between DOHMH/OPWDD and criminal justice system stakeholders (e.g., attorneys, parole, court) and community treatment providers, including housing providers and alternative to incarceration programs.

· Working with the Director of Social Work or assigned supervisor to provide input into program design and policy formation.

· Participating in program planning with regard to community health programs and facilities and discharge planning.

Preferred Skills

· Experience assessing for, submitting, and coordinating AOT referrals

· General understanding of OPWDD eligibility and services

· Clinical experience working with individuals with mental illness and/or neurodevelopmental disabilities

· Clinical experience working in with individuals involved in the criminal legal system

· Experience facilitating communication between stakeholders, including court personnel

· Excellent organizational skills

· Excellent interpersonal skills and ability to work collaboratively with other disciplines, including NYC Department of Correction staff

· Strong data tracking skills

· Master’s degree in Social Work with active NY State license in good standing

· 6 years of community-based case management or social work experience

· At least two years of post-license experience

Minimum Qualifications

1. Master’s Degree in Public Administration, Public Health, Health Care Specialization, Social Work, Psychology or related field; and
2. Three (3) years of increasingly responsible experience in hospital or regulatory agency administration, with particular emphasis on development and evaluation of mental health delivery services; or
3. Possession of a valid license and current registration to practice in a mental health field issued by the New York State Education Department (NYSED).

Pay: $78,000.00 - $88,000.00 per year
