What will you be doing:
We are looking for an experienced, talented, and compassionate Lead Home Care Social Worker for a full-time position based in the South Shore area.
The Lead Home Care Social Worker provides medical social services to home care patients/clients and/or family members reflective of the philosophy, mission and standards established by the Agency. The Lead Home Care Social Worker spearheads, supports and develops the home care social work program. You will conduct initial psychosocial assessment of the patient/client and family
What we’re looking for in you:
- Develops patient/client’s plan of care and patient/client’s response to treatment and his/her adjustment to their care plan.
- Ability to work effectively under pressure and in various work environments
- Ability to work independently with minimum supervision
- Ability to be organized and flexible with good time management skills
- Current Massachusetts State License (in good standing)
- 3 plus years of experience
- Masters required (LICSW or LCSW)
- Previous community health experience preferred
- Availability to work as as necessary including weekends and evenings
