RESPONSIBILITIES:

The Hospice Social Worker will work as part of a home based interdisciplinary team providing counselling and targeted interventions for a population of homebound patients and their caregivers who are receiving Palliative or Hospice services through NVNA and Hospice. Conducts initial psychosocial assessment of the patient/caregiver contributing to the Agency's plan of care. The Hospice Social Worker role strives for optimal patient outcomes with improved functioning and an increased ability to self-manage their bio- psych-social challenges. To provide medical social services to Hospice patients and/or family members reflective of the philosophy, mission and standards established by the Agency. This is a Full-time position, Monday - Friday working 8 hour days with a commitment of one 8 hour weekend rotation every 8-10 weeks.

Qualifications:

• Current MA license, in good standing
• Masters required (LICSW or LCSW)

Experience/Skills

• 3 plus years of social work experience required
• Previous community health experience preferred
• In-depth knowledge of social service skills and resources
• Good communication skills, both verbal and written
• Ability to harmoniously interact with patients/clients, families, co-workers
• Ability to work effectively under pressure and in various work environments
• Ability to be available as necessary including weekends and evenings
• Ability to work independently with minimum supervision
• Ability to be organized and flexible with good time management skills
