Complex conditions like COPD and congestive heart failure (CHF) are incredibly challenging to manage. People who are already struggling with isolation and the burden of these conditions become increasingly frustrated and disengaged, eventually falling through the cracks and landing in the ER. This cycle perpetuates poor health outcomes and excessive medical costs.

Where you’ll see your impact:
• Together we will provide the tools, education, and support to patients with complex medical needs to be able to live their best lives, as free from disease as possible
• We will do so in the comfort of their homes, avoiding costly and potentially damaging hospital care where appropriate, and helping to slow the progression of disease and disability
What you’ll be responsible for:
• Remote assessment and treatment of patients with COPD, asthma, and comorbid conditions including congestive heart failure
• Communicate with patients to understand their case and provide education to help improve outcomes
• Maintain high patient satisfaction scores
• Maintain timely accurate entry of patient interactions in the EMR (Athena)
• Supervise the performance and review of remote spirometry
• Participate in continuous process improvement and contribute to innovation
This may be a fit if you have:
• Holds a New York license for Nurse Practitioner
• Both pediatric and adult experience is strongly preferred
• Outstanding communication skills, including the ability to be personable and supportive of patients
• Experience providing telemedicine care to patients
• Proficient computer and internet navigation skills
• Passion for innovation and process improvement
• Experience leading, educating, and mentoring non-licensed team members
• Available to work minimum of 10 hours/week
First appt - 30 minutes $40 per visit
Follow-up - 20 minutes $30 per visit
