The Personal Care Assistant is responsible for administering quality care to patients by assisting with the patient’s physical and emotional comfort, and promotes the patients safety and progress toward independent living. This is performed in accordance with physician orders, plan of care, agency policy and under the direction and supervision of the Registered Nurse.

WHAT MAKES NCP DIFFERENT?

As a non-Medicare provider, NCP provides unique care experiences offering increased flexibility to our employees. At NCP our nurses are paid by the hour and not by the visit, allowing them to enjoy more time and deeper connections with patients. Our nurses help our patients by providing skilled care in the home. Each patient is unique in their needs, so in turn each RN has unique responsibilities in caring for their patient.

Responsibilities

• Provides hands on care to patients in their home as ordered by written plan of care.
• Reviews, understands and follows the HHA plan of care for each individual patient assigned on a daily basis to ensure appropriate care is provided.
• Maintains records of patient care, condition, progress, and problems in order to report and discuss observations with the registered nurse or appropriate supervisor/case manager.
• Works closely with Case Manager and supervising nurse(s) to ensure patient needs are being met.
• Completes all documentation in patient’s electronic medical record (EMR) on day of service.
• Reports any changes in patient’s condition and needs to the registered nurse or appropriate supervisor/clinical nurse manager.
• Assists patients and maintains patient’s physical and emotional comfort, and promotes client safety and progress toward independent living.
• Maintains a clean, safe, healthy environment by following basic infection control techniques.

Qualifications

• Complete and pass required training by state.
• Ability to exercise initiative.
• CPR Certificate/First Aid Certificate if required by state regulation
• Valid Driver’s License
• Current Automobile Insurance that meets NCP’s minimum requirement for coverage
