Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for receiving referrals and completing field visit. Your role will involve assessing client needs, developing care plans, coordinating services. Duties:
- Conduct comprehensive assessments of clients' needs
- Develop individualized care plans in collaboration with clients and their support systems
- Coordinate and monitor the delivery of services to ensure they align with clients' care plans
- Collaborate with community resources, such as social service agencies and healthcare providers, to connect clients with necessary services
- Maintain accurate and up-to-date case records, documenting all interactions and progress made
Qualifications:
- Bachelor's degree in social work, sociology, psychology, nursing or gerontology or related field or year for year job experience or any combination of education and related experience may be substituted for bachelors degree.
- Previous experience in case management or related field
- Strong communication skills, both verbal and written
- Ability to work effectively as part of a multidisciplinary team
- Excellent organizational skills and attention to detail
