Responsibilities\xc2\xa0 \xc2\xa0\xc2\xa0\xc2\xa0\xc2\xa0

• Implementation of physician\xe2\x80\x99s prescribed plan of care
• Patient assessments and coordination of care\xc2\xa0\xc2\xa0\xc2\xa0
• Administration of prescribed medication, treatments, and therapies\xc2\xa0\xc2\xa0\xc2\xa0
• Compliant, timely documentation of care provided in system of record\xc2\xa0\xc2\xa0\xc2\xa0
• Health promotion, teaching, and training of family members\xc2\xa0\xc2\xa0\xc2\xa0
• Responsible for accepting and writing verbal orders from physicians as required and for obtaining co-signature of Case Manager on all verbal orders\xc2\xa0
• Follow up with physician, client, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition\xc2\xa0\xc2\xa0\xc2\xa0\xc2\xa0
• Performs other appropriate duties as assigned\xc2\xa0

Qualifications \xc2\xa0\xc2\xa0\xc2\xa0\xc2\xa0

• Must have and maintain an active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice\xc2\xa0\xc2\xa0\xc2\xa0
• Current CPR certification from American Red Cross, American Heart Association, or BLS\xc2\xa0\xc2\xa0\xc2\xa0
• TB skin test (current within last 12 months)\xc2\xa0\xc2\xa0\xc2\xa0
• Six months prior hands-on nursing experience preferred but not required\xc2\xa0\xc2\xa0\xc2\xa0
• Must have reliable transportation\xc2\xa0\xc2\xa0\xc2\xa0
