Position Qualifications:

The ideal candidate will have a Degree in Social Work with prior experience in a long-term care, or rehabilitation hospital setting. Prior experience working with the elderly population preferred. Candidates should possess strong discharge planning skills, excellent organizational and communication skills, ability to handle multiple priorities efficiently, strong customer services skills.
