Northwest Children's Home is Hiring a Case Manager PRTF Near Lewiston, ID

SCHEDULE:

10 hour shifts, 4 days on/4 days off. Day Shift. Must be available to work weekends and holidays.

All the children in our care have experienced serious trauma. Because of that, they have some intense behavioral issues. They come to us for the therapeutic treatment plans our teams create and implement.

RESPONSIBILITIES:

To ensure the smooth, cooperative functioning of the treatment program by serving as a member of the Program Treatment Team (comprised of the Program Manager, Program Therapist, and Case Manager) with the collaborative goal of integrating clinical, milieu, and direct-care efforts and issues.

Foster a Family-Centered approach in working with the family members of a youth. Obtain and compile information from various sources and maintain a comprehensive record on each youth. Serve as an advocate for the resident during the youth's placement including the assurance of health, safety and residents' rights.

Coordinate care by serving as a liaison between members of the multidisciplinary team including the youth, family, contractors, therapists, directors, and coordinators.

MINIMUM QUALIFICATIONS:

  • Bachelor's Degree in Human Services, Social Work, or Nursing.
  • A minimum of one year of childcare experience required.
  • Valid state-issued driver's license.
  • Must be at least 21 years of age.
  • Legally authorized to work in the United States and able to provide verification.
  • Able to pass required background checks.
  • Within two months of hire date, completion of in-service training per agency policy.
  • Internal applicants must be current in required agency trainings.

PREFERRED QUALIFICATIONS:

  • Paid experience dealing with children/youth in groups.
  • Work effectively with students, coworkers, community members, and community agencies.
  • Excellent verbal and written communication skills.
  • Demonstrated ability to accomplish the performance tasks indicated.
  • Trained in Trauma Informed Care (TIC), or is able to attend TIC training immediately, and is actively engaged in furthering their TIC knowledge and training.

ESSENTIAL FUNCTIONS (for illustrative purposes):

  • Facilitate the admission and referral process, working closely with the Medical Director and Clinical Services Director and presenting referrals in a weekly team meeting.
  • During the evaluation period, ensure that all relevant data is collected including psychological evaluations, psychiatric reports, school data, medical information, prior treatment records, and other relevant records.
  • Schedule and conduct multidisciplinary meetings including those for the master treatment plan, treatment plan reviews, discharge meetings, and other case conferences as necessary. Submit written reports of these meetings within established time frames.
  • Maintain current, comprehensive case records of each youth, working cooperatively with the therapist, program staff, other agency departments, and Clinical Records Department.
  • Update team members of changes in case status including guardianship, caseworkers/parole officers, addresses, phone numbers, approved contacts, and discharge plans.
  • Serve as a resource to the multidisciplinary team members in relation to accurate and effective evaluation, planning, implementation, and accountability for each youth's program.
  • Ensure weekly and monthly client-staffing meetings, as indicated in policy, are completed and documented in a timely manner.
  • Notify and submit incident reports to guardians, internal staff, and stakeholders within established period.
  • Provide direct service to youth, including advocacy, discipline, instruction, and information giving.
  • Offer youth a nurturing and supportive relationship while communicating a clear set of values and expectations.
  • Maintain an understanding of agency and program treatment philosophy, policies and procedures, and conduct case management within these parameters.
  • Maintain consistent communication with each youth's parents/guardians and referring agency regarding treatment program, progress, incidents, and significant developments occurring during placement. Respond to questions and concerns as they arise.
  • Conduct personal and professional phone calls daily with youth to ensure they are staying in contact with supportive persons outside the agency.
  • Coordinate youth visits with outside agencies and family.
  • Ensure completion of all intake and discharge documentation including the completion of youth inventories within the required period.
  • Coordinate discharge planning and aftercare for youth.
  • Work efficiently and effectively as a member of a professional team.
  • Complete all other duties as assigned by supervisor.
