Job Description
Engages and provides care and support to Behavioral Health clients, patients, and/or families. Performs psychosocial assessment and care management. Plans, implements, and documents care and treatment. Collaborates with clinical and non-clinical team members and supports multidisciplinary team.
Job Responsibility
1. Assesses clients and conducts psychosocial assessment with family, significant others, and/or professional collaterals to determine patient conditions and needs.
2. Provides specialized therapeutic interventions, individual and/or group psychotherapy, as needed, as well as crisis counseling and intervention services, as appropriate.
3. Provides complex care management including identification and linkage to community services and resources, and/or access or transfer to a higher level of behavioral health care (e.g. partial hospitalization, inpatient hospitalization) as appropriate, and ensures patient moves along care continuum in a timely manner.
4. Promotes patient compliance and adherence to treatment as well as liaises with patient providers for collateral treatment.
5. Provides patients and families with the opportunity to engage in care and treatment as well as participates in developing and implementing new/revised approved programs, initiatives, techniques and/or protocols.
6. Completes documentation in accordance with policies and procedures of the organization, state and Federal regulations.
7. Collaborates with multidisciplinary team consisting of healthcare teams, patients and families to understand social and emotional elements related to diagnosis and treatment .
8. Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions.
Job Qualification
• Master's Degree required or equivalent combination of education and related experience.
• Current License to practice as a Master Social Worker (LMSW), Clinical Social Worker (LCSW) or Marriage and Family Therapist (LMFT) in New York State required, plus specialized certifications as needed
•1-3 years of relevant experience, required.
