NORTHRIDGE HEALTH CENTER is Hiring a LPN/RN MDS Coordinator Near North Ridgeville, OH

Embassy Healthcare has an exciting MDS opportunity for a seasoned MDS Nurse looking to join the Northridge in beautiful North Ridgeville, OH!

This position is open to either an RN or LPN, but experience is required.

Job Description:

Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations and guidelines that govern the resident assessment, including the implementation of CAAs and Triggers.
Maintain and periodically update written policies and procedures that govern the development, use and implementation of the resident assessment (MDS) and care plan.
Develop, implement and maintain an ongoing process for the resident assessment/care plans.
Assist the resident and discharge planning coordinator (Social services) in completing the care plan portion of the resident’s discharge plan.
Oversee the daily Utilization Review/ Case Management Review meeting which includes the interdisciplinary team and is intended to foster daily communication of client needs and changes in status which may impact the plan of care.
Participate in facility surveys (inspections) made by authorized government agencies.
Participate per rotating schedule (at administrator’s direction).
