Case Manager for Homeless Services - Summary

NorthKey Community Care is seeking two full-time Case Managers for Homeless Services to work with clients experiencing homelessness to provide case management and care coordination services. A Bachelor’s Degree in a human services field is required.

Case Manager for Homeless Services - Qualifications

  • Bachelor's degree in an acceptable human services or behavioral health field of study from an accredited school is required.
  • At least 1 year experience working with individuals experiencing homelessness and/or working in the mental health field.
  • Excellent written and verbal communication skills.
  • A valid driver's license and the ability to operate a motor vehicle.
  • May 2023 and December 2023 graduates encouraged to apply.

Case Manager for Homeless Services - Responsibilities

· Engage clients experiencing homelessness in a structured, collaborative relationship which empowers them to gain necessary skills in a variety of areas related to recovery and self-management of their mental health and/or substance use diagnoses.

· Provide evidence-based care concerning complex cases of vulnerable individuals experiencing homelessness.

· Participate in staffing meetings and consultation with the team to assess care coordination needs of clients and encourage appropriate referrals to internal and external community services.

· Assess, plan, implement, and document care coordination services to assigned clients.

· Act as an advocate to clients to ensure they receive necessary services in the community.

· Develop knowledge and establish good working relationships with community support resources and services within the community.

· Maintain communication and positive multi-disciplinary working relationships with clinicians and other related professionals in the community. This will include making appropriate referrals, sharing clinical information within HIPAA guidelines, and providing clinically appropriate interventions and collaborations.

· Complete timely and accurate documentation within an electronic health record and other documentation as required of the position.

· Attend and participate in relevant continuing education and in-service training to improve professional competency and clinical intervention skills.

· Maintain flexible hours to meet organizational and client needs.
